甲状腺乳头状癌局部免疫状态的分析

摘　　要：目的分析甲状腺乳头状癌(PTC)的局部免疫状态。方法应用流式细胞术测定35例PTC患者、25例良性甲状腺病变的局部组织CD3+、CD4+、CD8+及NK细胞的含量。结果PTC局部癌组织的CD4+/CD8+比值显著低于良性组局部病变组织(0.81±0.26vs1.46±0.53,t=0.87,P<0.01),与良性组相比较而言,主要表现在PTC局部CD8+细胞水平显著升高(49.63±8.58vs29.32±5.30,t′=11.30,P<0.01),而两者间CD4+细胞水平的差异不具统计学意义(38.30±8.67vs40.34±6.77,t=0.98,P>0.05)。15例伴淋巴结转移PTC局部癌组织的CD3+、CD4+、CD8+、CD4+/CD8+、NK细胞水平与20例无淋巴结转移的PTC局部癌组织之间的差别无具统计学意义(P>0.05)。结论PTC局部细胞免疫功能呈明显的抑制状态,但局部抗瘤免疫效应的削弱与PTC是否发生淋巴结转移无关。Objective To analyze the local immune response in papillary thyroid carcinoma （PTC）. Methods The levels of CD3^＋ ,CD4^＋ ,CD8^＋ and NK cell in local tumor tissue of 35 cases of PTC and 25 benign thyroid lesions were detected by flow cytometry. Results Local tumor tissue of 35 cases of PTC and 25 benign thyroid lesions was collected. The CD4^＋/CD8 ＋ ratio of local tumor tissue of PTC was significantly lower than that of the tissue of benign thyroid lesions（ 0. 81 ± 0.26 vs 1.46 ±0. 53, t = 0. 87, P 〈 0.01 ）. Compared to local tissue of benign thyroid lesion, the level of CD8^＋ cell of local tumor tissue of PTC was significantly higher（49.63±8.58 vs 29.32 ±5.30, t＇ = 11.30, P 〈0.01 ）. But the difference of the level of CD^4＋ cell of local tissue between PTC and benign thyroid lesion was not statistically significant（38.30 ± 8.67 vs 40.34± 6.77, t = 0.98, P 〉 0.05 ）. The levels of CD3^＋ , CD4^＋ , CD8^＋ , NK cell and CD4^ ＋/CD8^＋ ratio in local tissue of 15 PTCs with lymph node metastasis were not statistically different from those of 20 PTCs without metastasis（ P 〉 0.05 ）. Conclusions The local cell - mediated immunity of PTC is in the status of obviously suppressed. But the local impaired immune response against carcinoma is not associated with lymph node metastasis of PTC.
