64例精神分裂症血脂异常的临床分析

摘　　要：目的：分析精神分裂症血脂异常及相关临床特征。方法：对64例首发精神分裂症检测血糖、血脂及TNF-α浓度,采用HO-MA-IR指数评估IR发生,并采用PANSS量表评定精神状况。结果：64例患者中,脂代谢紊乱占66.7%,主要以TG升高（21.9%）和LDL下降（40.6%）为主,TC和HDL值基本正常;TG升高者腰围值明显高于正常者（P〈0.05）,而BM I值、QTc值及病程组间无差异;LDL降低者在年龄,PANSS总分,P分,G分上均显著低于正常者（P〈0.05）;各种血脂浓度与血糖、血胰岛素、IR指数及TNF-α无明显相关性（P〉0.05）。结论：首发精神分裂症存在血脂异常,有必要建立有针对性的早期干预模式。Objective：This study was to analyse the dyslipidemia in first-episode schizophrenia.Methods：64 schizophrenics were measured the fasting blood glucoses and lipids concentrations,TNF-α,and were assessed for insulin resistance using HOMA and rated the positive and negative symptoms scales（PANSS）.Results： Dyslipidemia（66.7%） was present in first-episode patients,including 21.9%with elevated TG,40.6% with the lower LDL.The waistline of the elevated TG groups was higher than normal groups（P〈0.05）,but no difference was found in the index of BMI、QTc and durations.The lower LDL groups were lower in the years,P,G and the total of PANSS than nomal groups（P〈0.05）.There was no correlations between lipid concentrations and FBG,FINS,IR and TNF-α（P〉0.05）.TC concentrations were positively related to the TG and the LDL levels（r=0.33 and 0.45,p〈0.01）.Conclusions： The current data suggested that the high incidence of dyslipidemia occur in the first-epside schizophrenia and hinted we should build the early interference models to the dyslipidemia.
