摘 要:目的应用速度向量成像(VVI)技术分析正常儿童左心室短轴的运动特点。方法选取正常儿童42例,结合体表心电图,采集胸骨旁左心室短轴乳头肌水平连续3个心动周期的图像动态存储,运用VVI软件进行脱机分析,动态观察各节段室壁运动的速度方向,并测量收缩期峰值速度、应变、应变率大小及各指标达峰时间。结果前间隔速度最小,后壁速度最大,差别有统计学意义(P〈0.05);应变以前间隔最大,且与其它节段比较,差别有统计学意义,应变率以前间隔最大,与前壁、侧壁、后壁、下壁比较,差别有统计学意义(P〈0.01);各节段收缩期速度达峰时间、应变达峰时间、应变率达峰时间差别无统计学意义(P〉0.05)。结论正常儿童左室短轴运动有其相应的特点,速度向量成像技术可用于观察儿童心脏整体运动及评价局部心肌功能。Objective To explore characteristics in short axis of left ventricle in normal children using velocity vector imaging (VVI) . Methods 42 healthy children were enrolled in this study. Combined with surface electrocardiogram, parasternal short axis view at the papillary muscle level were acquired in three heart cycles using Acuson Sequoia instrument. VVI software was used for off-line analysis. Velocity vectors in short axis view were observed, and peak systolic velocity, strain, strain rate and time to peak systolic velocity, strain, strain rate in short axis view were obtained by VVI respectively. Results Velocity of anteror septum was the lowest among all the six segments, opposite to posterior segments. Strain in anterior septum were the statistical highest among all the six segments. Strain rate in anterior septum was the highest, and there was statistical differences between anterior septum and anterior wall, lateral wall, posterior wall, inferior wall. There were no statistical differences of time to peak systolic velocity, strain, strain rate among all the six segments. Conclusions There were some characteristics in short axis of left ventricle in normal children. VVI is able to describe both global cardiac motion and regional myocardial function of the left ventricle in normal children.
