早期乳腺癌的彩超表现及误诊病例分析

摘　　要：目的：探讨早期乳腺癌的彩超表现,进一步提高早期乳腺癌的彩超诊断水平。方法：回顾性分析经病理证实的25例早期乳腺癌的声像图表现及血流特征。结果：早期乳腺癌病灶可表现为实性结节、囊实性结节、呈导管扩张伴散在钙化点或伴管壁增厚、局部钙化点聚集无结节征象;结节样病灶大小平均0.84 cm;多数结节（86.4%）边缘模糊毛糙;72.7%的结节内部回声为不均匀低回声;结节后方回声衰减占27.3%;纵横比0.62～1.33,平均0.96,其中大于1.0者占22.7%;结节内血流显示率72.7%,按Ad ler分级法,以0～Ⅰ级为主;有2例病灶术前超声考虑良性病变,1例考虑纤维腺瘤,1例考虑乳头状瘤伴输乳管扩张。结论：乳腺内≤1 cm的病灶,边缘模糊毛糙、内部不均匀低回声（囊性病灶内壁不光滑、腔内含有不规则实性成分）、纵横比〉1.0等特征,应高度怀疑恶性病灶。相对于血流分布特征来说,声像图特征对于诊断早期乳腺癌更重要。Objective： To explore the diagnostic ability of early breast cancer. Methods： Reviewed the ultrasonic characteristics of 25 patients with early breast cancer who confirmed by pathology. Restilts： Manifestations of early breast cancer were solid, blended echoes, duct ectasia with scattered calcifications, tube wall thickening, partial nodular calcifications without signs of aggregation. Average size of masses was 0. 84cm. The most of masses were spinous margin and the inner echoes were hyper dispersion. The posterior echoes attenuated were 27.3 %. The average longitudinal - transverse diameter ratio were 0. 96 ,the percentage of ratio 〉 1 ,was 22. 7%. According to Adler,blood flows were most in 0 - 1 degree. 2 patients were misdiagnosed as fibroadenoma and papilloma. Conclusion：The masses of ≤ 1 cm in size, possessing the ultrasonic characteristics of spinous margin and the inner echoes are hypoheterogeneous and longitudinal - transverse diameter ratio 〉 1, are suspected malignant nodules.
