摘　　要：目的为手前臂皮肤伴肌腱缺损修复提供一种新的有效的手术方法。方法在对以膝降动脉为蒂的大收肌腱复合组织瓣修复手部肌腱伴皮肤（骨）缺损的新术式进行解剖学研究的基础上，临床应用吻合血管的大收肌肌腱复合组织辫游离移植修复手前臂皮肤肌腱缺损3例。结果全部病例术后皮辨全部成活，随访3～14个月，平均5．4个月，根据手部功能评价为优1例，良1例，可1例。结论应用吻合膝降血管的大收肌肌腱复合组织瓣游离移植修复手前臂皮肤肌腱缺损的术式具有供区隐蔽创伤小等优点，此手术为手前臂皮肤肌腱缺损修复提供了一种良好的方法。Objective To offer a new and effective operation means for repairing hand or forearm skin accompanying muscle tendon defect. Methods On the basis of anatomical investigation about a new operation method which repairs the hand or forearm skin accompanying muscle tendon defect hy using the great adductor muscle tendon composite tissue flaps with descending genicular artery as pedicle ,3 cases with hand and forearm skin and tendon defects were successfully repaired by the clinical application of freeing and grafting the great adductor muscle tendon and composite tissue flap. Results In all eases ,the skin flaps were all alive after operation. The follow - up visits lasted for 3 to 14 months and the average one was 5.4 months. According to the functional evaluation of hands in the follow - up visits, 1 case was cxeeUent, 1 ease was fine and I case was so - so. Conclusion A new operation method by freeing and grafting the great adductor muscle tendon composite tissue flap with descending genieular vascular anastomosis has the merits of more concealed donor site, much smaller trauma, and so on. This new way also offers a favorable operation method to the repairment of hand or forearm skin accompanying muscle tendon defect in clinical application.
