均衡式三切口白内障超声乳化术对角膜散光的影响

摘　　要：目的:评价白内障超声乳化术中采用均衡式三切口对角膜散光的影响。方法:收集老年性白内障患者72例,术前角膜曲率K1与K2的差值≤0.50D,随机分为常规巩膜隧道切口组(A组)与均衡式三切口组(B组),于术后1wk;1,3mo进行视力及屈光状态检查。结果:A,B组术前角膜散光0.27±0.15D,0.29±0.16D,术后1wk;1,3moA组为0.70±0.34D,0.62±0.27D,0.58±0.26D;B组为0.38±0.22D,0.35±0.19D,0.31±0.15D两组角膜散光术前比较差异无显著性,术后各时段比较差异有显著性意义。两组散光随时间推移而减少,但B组始终小于A组。结论:均衡式三切口白内障超声乳化术可明显减少手术源性角膜散光。AIM： To evaluate the balanced three incisions on corneal astigmatism after phacoemulsification METHODS： In a prospective clinical study, 72 senile cataract patients were divided into two groups randomly： group A received superior scleral tunnel incision, group B received balanced three incisions. Patients were examined preoperatively and 1 week, 1 month and 3 months postoperatively by using slit-lamp microscope and keratometry, and visual acuity was also performed. RESULTS= Preoperatively, the mean astigmatism was 0.27 ± 0.15D in group A, 0.29 ± 0.16D in group B. At 1 week, 1 month, and 3 months postoperatively, the mean astigmatism in group A was 0.70 ± 0.34D,0.62 ± 0.27D, 0.58±0.26D; in group B was 0. 38 ± 0. 22D,0. 35 ± 0.19D, 0. 31 ± 0.15D. The difference between group A and group B was significant. CONCLUSION： Balanced three incisions induced the least astigmatism.
