经肛门拖出双吻合器保肛术治疗低位直肠癌的应用体会

摘　　要：目的评价直肠拖出采用双吻合器技术实施保肛低位直肠癌切除手术的疗效。方法对25例低位直肠癌经肛门拖出双吻合器保留肛门括约肌手术的病例资料进行分析。结果25例低位直肠癌患者均保留了有控制大便功能的肛门,术后发生切口感染2例,吻合口漏1例,无手术死亡,随访1～3年,有2例局部复发。结论经肛门拖出双吻合器保肛术治疗低位直肠癌具有操作简单、安全可行等优点,能提高患者的生存质量。Objective To evaluate the effect of pull-through combined with double stapling technique for sphincter preservation operation in low rectal carcinoma. Methods Pull-through combined with double stapling technique for sphincter preservation operation was performed on 25 patients with low rectal carcinoma form May 2004 to December 2007. The clinical data were analyzed retrospectively. Results Fecal continence was preserved successfully in all patients. There was no operative death. Only one case got anastomosis leak and 2 cases had incision infection after operation. Patients were followed-up for 1-3 years, and two pa- tients had local recurrences. Conclusion Pull-through combined with double stapling technique for sphinc- ter preservation operation is simple, safe and feasible for low rectal carcinoma, and enhance the quality of life.
