小切口手法劈核联合人工晶状体植入术在硬核白内障囊外摘出术中的应用

摘　　要：目的探讨硬核白内障小切口手法劈核联合人工晶状体植入术的手术方式及临床效果。方法对实施该手术的152例(175眼)Ⅳ级及Ⅴ级核白内障进行回顾性分析。分析手术方式的适宜性及治疗效果。结果术后视力明显提高,0.3及其以上者164眼占93.71%,0.05及其以上脱盲率为100%。平均角膜曲率术前与术后1周、1月、3月的差异无统计学意义(P>0.05);角膜内皮细胞密度术前与术后1周、1月、3月时相比差异有统计学意义(P<0.05),但术后1周、1月、3月之间的差异无统计学意义(P>0.05)。结论硬核白内障行小切口手法劈核联合人工晶状体术获得良好疗效。适合在基层医院开展。Objective To observe the clinic effect of small incision extracapsular cataract extraction with chop technique and IOL implantation. Methods A retrospective analysis was done on 152 cases（ 175 eyes）of cataract with IV or V degree nucleus, the fitness, clinic effect and complication of the operation were discussed. Results The eyesight of 164 eyes were 0.3 or above 0.3. That ocupied 93.71%. The eyesight above 0.05 oeupied 100%. Postoperative examinations at 1 week, 1 month, and 3 month, the difference of changes in the corneal curvature were not significantly changed preoperatively, 1 week,1 month and 3 months after surgery. （P 〉 0.05 ）. A significant change in corneal endothelium cell was found between before and after the surgery （ P 〈 0.05 ） , while no significant change in corneal endothelium cell was found among postoperative examinations at 1 week, 1 month, and 3 month （ P 〉 0.05 ）. Conclusion Small incision with chop technique extracapsular cataract extraction and IOL implantation was an effective way to cure cataract with hard nucleus. The method was easy to use and fit to the basic hospital.
