基质金属蛋白酶-2和PTEN在贲门癌中表达及临床意义

摘　　要：目的分析基质金属蛋白酶-2(MMP-2)及PTEN在贲门癌中的表达及与临床参数之间的关系。方法选择贲门癌手术切除标本86例,癌旁非肿瘤胃组织(离癌灶边缘>5cm)40例,二步法免疫组化检测MMP-2及PTEN表达,并分析2者与各临床参数之间的关系。结果MMP-2在贲门癌和癌旁组织阳性表达率分别为60.5%和40.0%(P<0.05),其表达与肿瘤浸润深度、淋巴结转移、临床分期呈正相关(P<0.01)。PTEN在癌旁组织和贲门癌组织中的阳性表达率分别为97.5%、55.8%(P<0.05),PTEN在贲门癌中的表达与肿瘤浸润深度、淋巴结转移及临床分期呈负相关(P<0.05,P<0.01)。结论PTEN缺失及MMP-2表达增加在贲门癌侵袭、转移中发挥重要作用。联合检测PTEN与MMP-2,有助于了解贲门癌的发展和评估贲门癌患者的预后。ObJective To analyze the relationship between expression of MMP-2 and PTEN and clinical characteristics in gastric cardia carcinoma. Methods Consecutively selected resection specimens of 86 cases with gastric cardia carcinoma,40 specimens of paraneoplastic gastric noncancerous tissues （the distance to cancer border more than 5 cm） were selected as normal control group. The expression of MMP-2 and FIEN was detected by i mmunohistoehemical,method. The rela- tionship between expression of MMP-2 and PTEN and elinieal eharaeteristics was eollected and analyzed. Results The posi- tive expression rate of MMP-2 in carcinoma of the gastric cardia and normal control tissue was 60.5% and 40.0% respec- tively （ P 〈 0.05 ）. The expression of MMP-2 was positively eorrelated with infiltration degrees, the number of lymphatie metastasis and elinieal staging （ P 〈 0.01） .Additionally,the positive expression rate of FIEN was 55.8% and 97,5% in carcinoma of the gastrie eardia and normal tissues respectively （ P 〈 0：05）.The expression of PTEN was negatively eorre- lated with infiltration degrees, the number of lymphatie metastasis and elinieal staging （ P 〈 0.05, P 〈 0.01）. Conclusion The absence of PTEN and the over expression of MMP-2, whieh may play an important role in the invasion and metastasis of gastric eardia eareinoma. Furthermore,combination detected expression of PTEN and MMP-2 eould be helpful to compre- hend the development and evaluate the prognosis.
