摘 要:目的:分析膀胱肿瘤彩色多普勒超声造影的血流分布特征,探讨其在膀胱肿瘤的分期和分级诊断中的应用价值。材料和方法:对30例38个临床确诊并预行经尿道切除或膀胱全切的膀胱肿瘤患者进行彩色多普勒超声造影检查。比较造影前后肿瘤血流显示强度的变化,分析肿瘤的血流分布特点以及造影增强过程中时相变化与病理分期、分级和肿瘤大小的相关性。结果:注射造影剂后,膀胱肿瘤血流显示率为100%;肿瘤<1.0 cm组与>1.0 cm组其血流分级有明显差异(P<0.01)。肿瘤病理分级与分期与血流分级无统计学意义(P>0.05);病理分期高的T2、T3期肿瘤开始增强、达峰时间较T1期为早,而且增强持续时间短,二者之间三个参数均有显著统计学意义(P<0.01);肿瘤病理分级高的(Ⅱ、Ⅲ级)开始增强、达峰时间存在较早现象,但无统计学意义(P>0.05),但增强持续时间短,二者之间具有统计学意义(P<0.01)。结论:彩色多普勒超声造影显著提高膀胱肿瘤的血流显示率,但其血流分布特征尚不能预计膀胱肿瘤的病理分期和分级。Purpose: The purposes of this study were to evaluate the feasibility of microvascular imaging using contrast-enhanced color Doppler uhrasonography ( CE CDU ) in bladder cancer, and to discuss its application in staging and grading bladder cancer. Materials and Methods: A total of 30 patients with 38 tumors selected as the candidate of transurethral resection or cysteetomy underwent CE-CDU. The signal of tumor blood flow before and after contrast (SonoVue) administration were compared, The characteristics of tumor perfusion and pattern of enhancement were correlated with the pathological stage and grade of the tumor. Results: Blood signal of tumor was found increased by SonoVue in all patients. The difference of blood signal after SonoVue administration was statistically significant(P 〈0.01)between small ( 〈 1.0cm) tumor and bigger( 〉 1.0cm) one. No correlation could be demonstrated between tumor pathologic grade, stage and intensity of blood signal. There was earlier but shorter enhancement and peak-time in T2 and T3 tumors than in T1 stage. High grade tumors ( Ⅱ, Ⅲ ) tended to have earlier enhancement and peak time but the difference was not statistically significant. Nevertheless, the duration of enhancement was longer in high grade tumor than low grade ( P 〈 0.01 ). Conclusion : CE CDU is helpful in characterizing the blood perfusion in bladder cancer, but not sensitive enough to stage and grade that tumor.
