脑膜癌病的临床和脑脊液细胞学特征4例分析

摘　　要：目的探讨脑膜癌病的临床和脑脊液细胞学特征及诊断价值。方法总结分析4例脑膜癌病的临床及脑脊液细胞学特征。结果例1为慢性粒细胞白血病急性变,例2为乳腺癌,例3和例4为肺癌脑膜转移。临床主要表现头痛、呕吐,同时有脑神经和脊神经受损症状。脑脊液检查颅压高,细胞数、蛋白轻-中度升高,糖、氯化物明显降低。脑脊液细胞学检查均发现肿瘤细胞。结论脑膜癌病主要表现为慢性颅高压症状。脑脊液细胞学检查是脑膜癌病生前的确诊方法。Objective To discuss clinical feature and cytological changes of cerebrospinal fluid （CSF） in meningeal careinomatasis. Meathods The clinical features and cytological changes of CSF of four meningeal carcinomatasis patients were analysed in combination with review of the literature. Results Four cases of meningeal carcinomatasis：one case was chronic myelocytic leukemia,with acute change;another case was postoperative breast cancer and Another two cases were lung cancer with meningeal carcinomatasis.Headache and vomiting were main clinical performance of the four meningeal earcinomatasis patients.In CSF of most patients with this disease there was slight-middle increase in protein and amount of cells,decrease in glucose and chloride.Cancer cell were found in the cytological examination of CSF. Conclusions The cytological examination of CSF is the most important evidence for the diagnosis of meningeal carcinomatasis.
