消化道巨大息肉的内镜下治疗

摘　　要：目的对胃肠道巨大息肉的内镜下治疗进行临床研究,探讨内镜下治疗胃肠道巨大息肉方法的选择及其安全性和有效性。方法内镜下依据息肉大小及形态选择合适的治疗方法,包括高频电切除术,单纯用尼龙圈套扎术和尼龙圈套扎后电切术。结果29例患者的29枚胃肠道巨大息肉均在内镜下治疗成功,高频电切除巨大息肉16枚,出血3例,出血率为18.75%;尼龙圈套扎后高频电切除息肉10例,单纯尼龙圈套扎巨大息肉3例,均无出血。所有患者术后1个月、3个月、6个月、1年复查,均无复发,有效率100%。结论内镜下治疗胃肠道巨大息肉是一种简单、安全有效的方法,尼龙圈套扎后高频电切息肉能有效预防出血和穿孔,特殊情况下单纯尼龙圈套扎,可以治疗胃肠道巨大息肉,但很难收集到整瘤体送病理检查。Objective To evaluate the efficacy and outcomes of endoscopic treatment for large alimentary canal polyps. Methods We ehosed suitable endoscopic treatments according to the size and the shape of the polyps. The methods including high frequency electric resecfion, snare of nylon, high frequency electric resection after snare. Resealts Totally 29 case large alimentary canal polyps were treated endoseopieally. The ease of high frequency electric resection was 16, three of them is bleeding, the rate of bleeding oceured was 16 polyps 18.75%; the case of high frequency electric resection after snare was 10, all of them had no bleeaing after the treatment. All patients had been revisited in the first month,third,sixth and one year, no recrudescence ,the rate of curative effect was 100%. Conclusion Endoscopic treatment for large alimentary canal polyps is a simple and safe method.
