摘　　要：目的探讨C反应蛋白(CRP)基因+1444C/T多态性与缺血性脑卒中的关系。方法选择286例缺血性脑卒中患者,根据脑动脉狭窄情况分为狭窄(136例)和无狭窄(150例)两组。同期健康体检者140例为对照组,应用PCR-RFLP方法检测受试者的CRP基因型,同时测定血清高敏C反应蛋白(hs-CRP)浓度。结果狭窄、无狭窄脑卒中组血清CRP浓度均显著高于对照组(P均<0.01),而这两组间CRP浓度差异无统计学意义。CRP基因+1444C/T多态性在各组人群中的分布无统计学差异(P>0.05)。对照组CT基因型患者血清CRP浓度显著高于CC基因型患者[1.59(0.73-4.62)mg/L vs 0.67(0.10-1.81)mg/L,P<0.01],但卒中组不同基因型个体间hs-CRP浓度无统计学差异。结论血清hs-CRP浓度与缺血性脑卒中相关,而与卒中患者狭窄与否无关。CRP基因+1444C/T多态性与缺血性脑卒中无相关性,但其与健康人群血清CRP浓度有关。Objective To investigate the association between ＋ 1444C/T polymorphism of the CRP gene and risk for ischemic stroke.Methods Polymerase chain reaction-restriction fragment length polymorphism was used for the detection of CRP genotypes in 286 patients with ischemie stroke （ 136 with and 150 with no cerebral artery stenosis） and 140 controls. The serum hs-CRP levels were measured by routine method. Results Serum CRP concentrations in the two groups of patients with isehemie stroke were significantly higher than that of the controls （all P 〈 0.01 ）. However, no significant difference was found in ,Serum CRP concentrations between with and with no cerebral artery stenosis. The distribution of CRP gene ＋ 1444C/T polymorphism did not show significantly different between isehemic stroke and control groups （ P 〉 0.05）. In controls, the serum CRP levels were significantly higher in CT individuals than CC individuals [ 1.59（0.73 - 4.62） mg/L vs 0.67（0.10 - 1.81 ） mg/L, P 〈 0.01 ]. The CRP ＋ 1444C/T polymorphism did not influence CRP levels in patients. Conclusion The increased serum CRP levels is associated with isehemie stroke. CRP ＋ 1444C/ T polymorphism is not associated with ischemie stroke. However, the polymorphism may affect serum CRP levels in controls.
