摘 要:目的:观察睫状神经营养因子(CNTF)能否促进视神经(ON)损伤后在原中枢神经环境中再生,为临床救治提供理论依据和新的治疗途径。方法:选用成年日本青紫蓝兔22只,其中随机抽取两只做正常对照,不做任何处理;其余20只为实验组,构造视神经中度损伤模型(中号显微血管夹于视神经后3 mm处夹持20 s)。于损伤后即时、3 d、7 d、14 d各时间点给予右眼(实验眼)玻璃体腔内注射CNTF 2μl(1μg/μl),左眼(对照眼)于同等时间点注射等量双蒸水。于损伤后3 d、7 d、14 d、28 d分别取实验眼和对照眼视神经行常规HE染色观察视神经组织学变化;免疫荧光检测观察生长相关蛋白-43(GAP-43)在视神经的表达;同时将动物实验眼及对照眼于损伤前、损伤后即刻及处死前进行视觉诱发电位(F-VEP)检查以观察兔视功能变化情况,采用SPSS 13.0统计学软件包对数据进行统计学分析。结果:HE及免疫荧光检测实验组组织学变化明显好于对照组;F-VEP检查可见视神经损伤后即时闪光视觉诱发电位波形几近熄灭。伤后1周,两组潜伏期基本恢复至损伤前水平,振幅恢复缓慢,组间比较差异无统计学意义(P>0.05);2周以后两组间比较差异有统计学意义(P<0.01)。结论:睫状神经营养因子对视神经损伤有修复作用,可促进视神经轴突在原中枢环境中的再生。Objective:To observe the effect of Ciliary Neurotrophic Factor (GNTF) promoting the regeneration of the optic nerve in the original centre environment after its injury, providing the theory basis and new therapy method for clinical treat and cure. Methods: Choose twenty - two Japanese cyanotic blue rabbits, two of them were normal contrast and others were experimented group randomly, establishing model of middle optic nerve injury (the optic nerve of each eye was crushed by microforceps for 20 seconds at 3 mm behind the eyeball). CNTF 2μl(1μg/μl) was injected into vitreous cavity of right eyes(experimented eyes)after injury, while distilled water(2μl)was done in left eyes(control eyes). The intravitreous injections were repeated respectively on 3, 7, 14 days after injury. The rabbits were sacrificed on 3, 7, 14, 28 days after injury and the changes of tissue morphology of optic nerve were observed by routine HE stain, meanwhile we detect the GAP - 43 expression in each optic nerve with immunofluorescence. F - VEP examinations were performed before, immediately and 3, 7, 14, 28 days after injury. The data were analyzed statistically with SPSS13.0. Results: The tissue changes of experimented eyes were lighter than the control eyes in HE and immunofluorescence stain. F - VEP Wave almost disappeared when the optic nerve was crushed. The latency of F - VEP recovered to the value before injury on the first week, the amplitude slowly recovered, showing no significant difference between Two groups (P 〉 0.05). The amplitude of treated group was significantly greater than that of control group on 2 weeks after injury (P 〈 0.01) . Conclusion. CNTF can effectively promote the recovery of optic nerve; meanwhile it can advance the regeneration of axons in the original centre environment.
