卒中单元治疗急性脑血管病的临床对照研究

摘　　要：目的探讨卒中单元治疗模式对急性脑血管病(ACVD)的效果。方法将符合标准的673例ACVD患者分为卒中单元治疗组(385例)及常规治疗组(288例)进行治疗。比较分析两组住院病死率、合并症发生率、近期临床疗效以及NIHSS、BI、OHS的差异。结果尽管两组在住院病死率上差异无统计学意义,但A组在近期临床疗效方面及NIHSS(3.51±4.46VS4.76±7.33)、BI(84.31±21.60VS79.46±16.70)、OHS(1.27±1.31VS1.55±1.12)等评分上均明显优于B组(P<0.01),合并症发生率两组为24.14%VS31.60%,A组显著低于B组(P<0.01)。结论卒中单元医疗模式治疗ACVD有确切疗效,它能明显提高治愈率、减少并发症和致残率,提高患者的生活质量,是适合我国的ACVD的治疗模式。Objective To investigate the curative effects of stroke unit for acute eerebrovaseular disedses（ACVD）pafients. Methods 673 patients in hospital who metched the standard were entered the study. All patients were assigned either into stroke unit group（Group A, n = 385 ） or general ward group（Group B, n = 288 ）. The data of the patients of the two groups were compared. The key indexes recorded were mortality, complication, the short term clinical effect and other parameters observed were National Institutes of Health Stroke Scale （NIHSS ）, Barthel Index （ BI）, Oxford Handicap Scale （ ORS ）. Results There was no statistical difference with mortality between the two groups（ P 〉 0.05）, but the mean scores after treatment demonstrated significant difference（ P 〈 0.01） in NIHSS（3.51 ±4.46 VS 4.76± 7.33）,BI（84.31 ±21.60 VS 79.46± 16.70）,or OHS（1.27 ±1.31 VS 1：55 ± 1.12） .The short term clinical effect in Group A were better than those of Group B significantly（ P 〈 0.01 ）. The complications rate of Groups A were aless than those of Group B significantly （24.14% VS31.60%, P 〈 0.01）. Conclusion It was sure that treatment model of stroke unit is effective and safe in treating ACVD. The stroke unit may improve obviously curative rate,reduee complication,decrease the disability and increase the life level of patients. It is a perfect treatmnet model that fit to our national conditions.
