喹硫平合并氯硝西泮治疗阿尔茨海默病伴精神障碍的随机对照研究

摘　　要：【目的】研究喹硫平合并氯硝西泮治疗阿尔茨海默病（AD）伴精神障碍患者的疗效及不良反应。【方法】对68例AD伴精神障碍患者随机分为单纯服用常规剂量喹硫平组[（对照组，常规剂量100mg／d基础上适当加大至150-275mg／d）及常规剂量喹硫平合并服用氯硝西泮（0．5-3mg／晚）组（观察组）]，在治疗前及治疗4周后，采用阳性症状和阴性症状量表（PANSS）评定临床疗效，不良反应量表（TESS）评定不良反应。【结果】经治疗后对照组有效率为87．9％，观察组有效率为93．9％，两组疗效无显著性差异（P〉0．05），观察组副作用小于对照组（P〈0．05）。【结论】喹硫平合并氯硝西泮治疗AD伴精神障碍疗效显著且副作用小。[Abstract][Objective]To study the clinical efficacies and adverse reactions of clonazepam combined with quetiapine in the treatment of Alzheimer＇s disease（AD） with mental disorders. [Methods]Sixty-eight patients with AD were randomly assigned to two groups： the quetiapine group with normal dose（control group） and the clonazepam and quetiapine with normal dose group（observation group）. The clinical efficacies and adverse reactions were evaluated with PANSS and TESS after treatment for 4 weeks. [Results]The effective rate was 87. 9% in group of quetiapine （204.55±64.49mg/d） , and the effective rate was 93.9% in group of clonazeparn（0. 73±0.33mg/d） and quetiapine （181.82±61.00mg/d）. There were no statistical differences in the effects of both drug groups （ P 〉0.05）, and the side effects of clonazepam and quetiapine with normal dose group were lower than those of quetiapine group with norreal dose （ P 〈0.05）. [Conclusion] Clonazepam combined with quetiapine is efficacious in the treatment of Alzheimer＇s disease with mental disorders, and has fewer side effects.
