小细胞肺癌的综合治疗

摘　　要：【目的】比较不同治疗方式对小细胞肺癌(Small cell lung cancer,SCLC)治疗生存率的影响。【方法】回顾分析393例SCLC临床治疗随访资料。其中88例广泛期SCLC采用化疗和/或放疗。局限期SCLC 305例分3组:52例为单纯化/放疗组,123例为化疗+手术+化/放疗组,130例为手术+化/放疗组,对各组1、3、5年生存率进行分析。【结果】局限期SCLC中,单纯化/放疗组1、3、5年生存率为63.4%、28.8%、7.6%,化疗+手术+化/放疗组为82.1%、56.1%、37.4%;手术+化/放疗组为77.7%、44.6%、27.7%。广泛期SCLC 1、2、3、5年存活率为17.0%(15/88)、10.2%(9/88)、0%(0/88)、0%(0/88)。与"单纯化/放疗组"相比,"手术+化/放疗组"和"化疗+手术+化/放疗组"病例的1、3、5年生存率均有提高,差异有显著性(P<0.05);"化疗+手术+化/放疗组"与"手术+化/放疗组"相比,3、5年生存均有提高,差异有显著性(P<0.05)。【结论】广泛期小细胞肺癌已经无手术治疗机会,采用单纯化/放疗保守治疗为主。局限期小细胞肺癌包含手术在内的综合治疗组效果优于单纯化/放疗组,手术前辅助化疗具有有益性和可行性。[ Objective] Compare results of combined surgical therapy and chemo- radiotherapy for small cell lung cancer （ SCLC ）. [ Methods] 88 cases extensive stage small cell lung cncer and 305 cases limited stage SCLC were treated. The 1, 3, 5 year overall survival rate of chemoradiotherapy group and combined surgical therapygroup were analysised. [ Results] For limited stage SCLC , the 1, 3, 5 yr survival rate for chemo - radiotherapy group was 63.4 %, 28.8 %, 7.6 %. The survival rate for preoprative chemotherapy combined surgical therapy group was 82.1%, 56.1%, 37.4%. The survival rate for postoprative chemotherapy combined surgical therapy group was 77.7 %, 44.6 %, 27.7%. For extensive stage SCLC , the 1, 2, 3, 5 yr survival rate for chemo- radiotherapy group was17.0% （15/88）, 10.2% （9/88）, 0% （0/88）, 0% （0/88） . There were significant difference between the groups. [ Condusions] Combined surgical therapy group has a better pmgnnsis than single chemo- radiotherapy group. Preoperation chemotherapy group had a better results than others.
