颅内生殖细胞瘤患者血清β-HCG水平与预后相关性研究

摘　　要：目的比较血清人类绒毛膜促性腺激素β亚单位(β-HCG)升高与β-HCG正常的颅内生殖细胞瘤患者的预后,研究血清β-HCG与生殖细胞瘤预后的关系。方法对72例经病理学确诊的颅内生殖细胞瘤患者,在治疗前全部行血清β-HCG检查,其中血清β-HCG正常者45例,β-HCG升高者27例,经过系统的治疗后,我们比较了两组患者的预后。结果在β-HCG升高的患者中,5年、10年生存率分别为93%(25/27)和89%(24/27),5年、10年无复发生存率均为81%(22/27);在β-HCG正常的患者中,5年、10年生存率分别为96%(43/45)和91%(41/45),5年、10年无复发生存率分别为84%(38/45)和82%(37/45)。在两组患者中,预后没有明显差异(P>0.05)。结论血清中β-HCG水平的高低与颅内生殖细胞瘤患者的预后没有明显的联系。Objective To study the relationship between β-human chorionic gonadotropin （β-HCG） level in the serum and the prognosis in the patients with intracranial germinomas. Methods The serum β-HCG level was determined before the treatment in 72 patients with intracranial germinomas diagnosed definitely as by pathological examination. The relationship between the serum β-HCG level and the prognosis in the patients with intraeranial germinomas was analyzed. Results Of 72 patients, 45 had normal level of serum β-HCG and 27 high level of serum β-HCG. The 5- and 10-year survival rates were 93%（25/27） and 89%（24/27） respectively, and the 5- and 10-year relapse-free survival rates were 81%（22/27） and 81%（22/27） respectively in the patients with high level of β-HCG. The 5- and 10-year survival rates were 96%（43/45） and 91%（41/45） respectively, and the 5-and 10-year relapse-free survival rates were 84%（38/45） and 82%（37/45） respectively in the patients with normal level of β-HCG. There were insignificant differences in the 5-and 10-year relapse-free survival rates, and 5-and 10-year relapse-free survival rates between the patients with normal level of serum β-HCG and the ones with high level of serum β-HCG （P〉0.05）. Conclusions There is insignificantly relationship between the prognosis and serum β-HCG level in the patients with intracranial germinomas.
