22例子宫奇异型平滑肌瘤的效果病理分析

摘　　要：目的探讨子宫奇异型平滑肌瘤的组织形态学特征及其诊断和鉴别诊断。方法采用肉眼、光镜观察及免疫组织化学方法对22例子宫奇异型平滑肌瘤进行组织病理学分析。结果22例子宫奇异型平滑肌瘤粘膜下肌瘤6例,肌壁间肌瘤12例,浆膜下肌瘤4例;镜下部分或全部胞核奇异,双核或多核易见,其中8例偶见核分裂像(≤1个/10HPF);全部瘤细胞胞质平滑肌肌动蛋白(SMA)染色阳性,呈肌源性表达。结论子宫奇异型平滑肌瘤是一种特殊的平滑肌瘤,临床表现是良性的,容易误诊,需与平滑肌肉瘤进行鉴别。Objective To investigate the histomorphological characteristics of uterine bizarre leiomyoma as well as its diagnosis. Methods Totally 22 leiomyomas cases were clinicoathologically analyzed. Results Submueous myoma, intramural myoma and subserous myoma were obsereved in 6, 12 eases and 4 cases respectively. Light microscopy showed that part of all the cell nuclei were bizarre, accompanied with double or multiple nuclei and the mitotic figures were occurred in 8 cases （≤1/10HPF）. Among 22 cases with immunohistochemical staining, markers indicating muscle cells in origin were positive expression in the bizarre cells and all control tumor cells. Conclusions Uterine bizarre leiomyoma is a kind of especial, benign and easily misdiagnosed leiomyomas, thus it needs to be differentiated with leiomyosarcoma in pathological diagnosis.
