摘  要:目的评价表观弥散系数(ADC)和相对脑血容量(rCBV)在脑胶质瘤的良、恶性诊断及分级中的价值。方法对9例低级别(Ⅰ~Ⅱ级)胶质瘤及13例高级别(Ⅲ~Ⅳ级)胶质瘤进行常规MR及弥散、灌注加权成像,分析肿瘤的ADC值、rCBV比值,并分析ADC值与rCBV比值间的相关性。结果低度恶性胶质瘤与高度恶性级胶质瘤之间ADC值差异有统计学意义[(12.73±1.40)]×10-4mm2/svs[(9.09±1.85)]×10-4mm2/s];低、高度恶性胶质瘤的最大rCBV比值分别为3.60±1.09和7.92±2.40,差异有统计学意义。ADC值与rCBV比值间呈负相关(r=-0.53,P<0.01)。结论结合常规MRI,ADC值与rCBV比值在脑胶质瘤良、恶性诊断及术前分级中有重要价值。Objective To assess the value of apparent diffusion coefficients (ADC) and regional cerebral blood volume (rCBV) in diagnosis of benign and malignant cerebral gliomas and in grading of the gliomas. Methods Nine patients of lowgrade ( Ⅰ- Ⅱ ) gliomas and 13 patients of high-grade (Ⅲ --Ⅳ) gliomas, underwent conventional MR imaging, diffusion and perfusion weighted imaging with Siemens Avanto 1. 5T MR unit. ADC value and rCBV ratio were recorded, and their correlation was analyzed. Results The mean value of the ADC was (12.73±1.40) × 10^-4 mm^2/s in the low-grade gliomas and (9.09±1.85)×10^-4 mm^2/s in the high-grade gliomas. The value of the rCBV ratio was 3.60±1.09 in the low-grade and 7.92±2.40 in the high-grade gliomas. There was significant difference between the low-grade gliomas and the high-grade gliomas. There was significant correlation between ADC and rCBV ratio (r=-0.53, P〈0.01). Conclusion ADC value and rCBV ratio can help to distinguish the benign and malignant cerebral gliomas, playing a very important role in grading of the gliomas.
