喉鳞癌患者血浆MMP-9及SCCAg与喉鳞癌生物学特性的相关性研究

摘　　要：目的：探讨喉鳞癌病人血清中基质金属蛋白酶-9（MMP-9）和鳞状上皮细胞癌抗原（SCCAg）的表达与喉鳞癌生物学特性的相关性。方法：选取经病理切片证实的喉鳞癌病人125例,良性喉部病变组79例,健康人血清42例,空腹抽取静脉血3m l检测MMP-9和SCCAg,并探讨两者与喉鳞癌生物学特性的关系。结果：MMP-9水平恶性组（128.75±50.89）ng/ml,良性组（79.81±18.86）ng/ml,对照组（73.43±12.34）ng/m l;良性组、对照组和恶性组比较均有统计学意义（P〈0.05）。SC-CAg水平恶性组（4.23±0.87）ng/L,良性组（0.99±0.51）ng/L,对照组（0.76±0.64）ng/L;良性组、对照组和恶性组比较均有显著的统计学意义（P〈0.05）。MMP-9在临床各期的阳性率为0%～100%,Ⅱ、Ⅲ、Ⅳ期与0、Ⅰ期间,病理分级G1与G2、G3间,有无淋巴结转移之间的表达均有显著的统计学意义（P〈0.05）。SCCAg在临床各期的阳性率为63.64%～100%,临床分期中0、Ⅰ期与Ⅱ、Ⅲ、Ⅳ期期之间的表达均有显著的统计学意义（P〈0.05）,而病理分级和有无淋巴结转移间的表达无显著统计学意义（P〉0.05）。结论：MMP-9及SCCAg均在喉鳞癌的表达有较高的敏感性,两者对喉癌的分期,转移及预后有较密切的联系。有可能成为喉癌诊断治疗的生物学指标。Objective： To study the relationship to the serum matrix metalloproteinase - 9 （ MMP - 9 ） and squamous cell carcinoma antigen （ SCCAg ） level of and biological characteristic of laryngo - carcinoma pa- tients. Method： 3ml blood sampling from 125 laryngo -carcinoma patients confirmed by pathological exami- nation ,79 benign laryngeal lesions patients and 42 healthy men for assessment of the MMP -9 level were performed on fasting condition. Result： Significant differences of serum MMP -9 level were observed between malignancy group, benign laryngeal lesions group and control group （ 128.75 ± 50.89 ） ng/ml, （ 79.81 ± 18. 86 ） ng/ml, （ 73.43 ± 12.34） ng/ml （ P 〈 0.05 ） ; There were significant differences of SCCAg level in malignancy group, benign group and control group （ 4.23 ± 0.87 ） ng/L, （ 0.99 ± 0.51 ） ng/L and （ 0. 76 ± 0.64 ） ng,/L（ P 〈0.05 ）. The positive rates of serum MMP -9 in clinical stage were 0 - 100 percent with statistical significance in clinical stage, pathological grading and lymph node involvement （ P 〈 0.05）. The positive rates of SCCAg were 63.64 - 100% with statistical significance between stage 11,111, IV and 0, clinical stage, pathological grade and lymph node involvement （ P 〈 0.05）. Conclusion ： The levels of serum MMP - 9 and SCCAg might play an important role in diagnosing laryngo - carcinoma, they might be related to clinical stage and metastasis and become the biological indicators.
