结直肠癌中TF和VEGF表达的相关性及其意义

摘　　要：目的探讨人结直肠癌组织中组织因子(Tissne Factor,TF)和血管内皮生长因子(Vascular EndethelialGrowth Factor,VEGF)表达的相关性及其与肿瘤临床病理特征之间的关系。方法用免疫组化ElivisionTMplus法检测40例结直癌组织中TF与VEGF的表达水平。结果TF、VEGF蛋白在结直肠癌中的表达与肿瘤生长方式、肿瘤原发部位、肿瘤级别高低均无明显差异(P>0.05);TF在淋巴结转移阳性组其蛋白阳性表达率明显高于阴性组(P<0.01),有无肝转移2组蛋白阳性表达率有明显差别(P<0.05);TF蛋白表达与TNM分期相关(P<0.05),与肿瘤侵润深度相关(P<0.01);而VEGF蛋白表达与肿瘤侵润深度、有无淋巴结转移及肝转移均无明显差异(P>0.05)。TF蛋白的表达与结直肠癌的临床病理分期相关,分期越高,阳性率越高(P<0.05),而VEGF蛋白的表达在Ⅰ、Ⅱ期与Ⅲ、Ⅳ期中阳性率分别为52.9%、86.7%,可见Ⅲ、Ⅳ期组表达要高于Ⅰ、Ⅱ期组,40例结直肠癌中TF和VEGF的蛋白表达呈正相关(rs′=0.517,P=0.001)。结论TF表达与结直肠癌侵袭转移有关,TF阳性表达组中VEGF阳性表达增强,两者呈正相关。联合检测TF和VEGF在结直肠癌中的表达对结直肠癌病情预后评估有一定意义。Objective To investigate the expression of Tissue Factor （TF） and Vascular Endothelial Growth Factor （VEGF） in colorectal carcinoma and their relationship with clinical pathological features. Methods The expression of TF and VEGF in 40 cases of colorectal carcinoma were detected by ElivisionTM plus immunohistochemical method. Results The expression of TF and VEGF were not correlated with the growth type, location and histologic type of colorectal carcinoma （P〉0.05）. The positive rate of TF expression was significantly higher in patients with lymph node than without lymph node （P〈0.01）,The positire rate of TF exprossion was higher in patients with liver metastasis than without （P〈0.05）, TF expression was significantly correlated with TNM stage and the depth of invasion（P〈0.05）. The expression of VEGF had no correlation with the depth of invasion, lymph node metastasis and liver metastasis （P〉0.05）. The incidence of VEGF expression in Ⅲ/Ⅳ stage was significantly higher than that in Ⅰ/Ⅱ stage （86.7% vs 52.9% ,P〈0. 05）. TF postive carcinomas presented a higher VEGF expression than TF negative carcinomas. There was a positive correlation between the expression of TF and VEGF（rs＇=0. 517,P=0. 001）. Conclusion TF expression was associated with the invasion and metastasis of colorectal carcinoma. The combination of detecting TF and VEGF expression is helpful to evaluate the prognosis of colorectal carcinoma.
