摘 要:目的探讨彩超筛查法对产前胎儿神经系统畸形的诊断价值,以便为临床处置提供决策信息。方法总结常规超声产前检查的妊娠18-40周连续病例8890例中疑诊有胎儿畸形,要求采用彩超筛查法,最后确定诊断的一组产前胎儿神经系统畸形声像图特征。结果8890例胎儿中,经引产或出生后证实的神经系统畸形35例,彩超筛查法对产前胎儿神经系统畸形诊断符合率为100%(35/35),其中有4例合并其他系统畸形漏诊。结论彩超筛查法对产前胎儿神经系统畸形的诊断具有重要的临床价值。但应指出,可疑病例与合并其他系统畸形检出的准确性尚应进一步提高。Objective To evaluate color Doppler US prenatal diagnostic value for fetal nervous system anomalies in order to make clinical effective remedies. Methods Suspected fetal anomaly cases in 8890 pregnant women with gestational age 18-40 weeks were referred to ultrasound consultation center of our hospital for routine obstetric ultrasound and color Doppler to make definite diagnosis. The fetal nervous system anomaly images were analysed. Results In 8 890 fetal cases, 35 cases with fetal nervous system abnormalities were confirmed by post-natal evaluation or abortion. Thirty five cases were verified on color Doppler US. Color Doppler US prenatal diagnostic coincidence rate of fetal nervous system anomalies was 100% (35/35) . Four cases with the other system anomalies were missed diagno- sis. Conclusions It is suggested that color Doppler US provides important valuable information for diagnosis of fetal nervous system anomalies, but suspected nervous system anomaly cases and the diagnostic accuracy of complicating the other system anomalies need further improvement.
