摘  要:目的探讨直接数字曲面体层技术在颌骨骨纤维异常增殖症诊断中的应用价值。方法回顾性分析6例7张经手术病理证实的颌骨骨纤维异常增殖症的直接数字曲面体层片的技术资料、临床特点和影像特征。结果上、下颌骨病变各3例,单骨发病者4例,多骨发病2例;多骨发病者累及上颌骨、颧骨。图像处理技术可以提高图像的可视性,但对诊断力的提高无影响。影像学表现分4型:透射性改变1例、阻射性改变2例、透射及阻射混合性改变2例、恶性肿瘤征1例。明显沿颌骨外形呈“发面团”样膨大、病变范围广泛、无明确边界是颌骨骨纤维异常增殖症的共同影像学特点。结论直接数字曲面体层技术是一种低辐射、操作简便、经济实用的新型影像学检查方法。骨纤维异常增殖症一般仅依靠直接数字曲面体层即可做出诊断,但对发生于下颌骨颏部的骨纤维异常增殖症,尚须结合其他片位;对于发生在上颌骨且累及多骨的骨纤维异常增殖症,进行CT检查是必要的。Obective To evaluate the diagnostic value of direct digital pantomography for fibrous dysplasia (FD) in jaw. Methods Six cases pathologically confirmed FD in jaw were reviwed. Results The age of all cases were from 3 to 51 with an average 22. 3 cases were maxilla and 3 were mandible. 4 cases were mono- osseous FD and 2 were poly-osseous FD involved maxilla and zygoma. Radiographic changes were divided to four types: radiolucency, radiopaque, mixed radiolucency and radiopaque and malignant tumor appearance. Expanding along the shape, unidentifiable borderline were characteristic radiographic appearance of FD in jaw. Conclusion The direct digital pantomography is an easy operating, inexpensive and useful new medical imag technique.
