大肠癌患者癌组织胃泌素表达与血清胃泌素水平的临床研究

摘　　要：目的：观察大肠癌患者血清胃泌素及肿瘤组织的ＧＡＳ表达情况。方法：采用放射免疫及免疫组化技术测定７０例大肠癌患者血清胃泌素水平。结果：胃泌素阳性表达率，中、低分化癌比高分化癌为高；粘液腺癌、印戒细胞癌、未分化腺癌较乳头状腺癌和管状腺癌为高，差异均非常显著（Ｐ＜０．０１）。结论：胃泌素阳性细胞过度表达可能是大肠癌分化不良的一个标志，且可能是肿瘤恶性程度增高的原因之一。血清胃泌素测定对诊断大肠癌灵敏度和特异性不高，但由于其升高为癌组织胃泌素阳性表达所致，与肿瘤的分化程度、恶性度、大小及分期密切相关。大肠癌病人术前血清胃泌素水平增高，则提示其肿瘤恶性程度较高，分期较晚，预后不良。Gastrin　content　　and　serum　level　in　70　cases　of　colorectal　cancer　were　determined　by　immunohistochemically　and　radioimmunologically.The　results　showed　that　the　positive　rates　of　gastrin　reaction　of　moderately　and　poorly　differentiated　cancers　were　significantly　higher　than　that　of　well　differentiated　cancer,and　that　the　in　mucinous　adenocarcinoma,signetring　cell　carcinoma　and　undifferentiated　adenocarcinoma　gastrin　presence　was　significantly　higher　than　in　pillary　and　tubular　adenocarcinoma(P<001).It　was　suggested　that　overexpression　of　gastrin　signifies　poorly　differentiated　colorectal　cancer.The　sensitivity　and　specificity　of　determing　serum　gastrin　level　for　colorectal　cancer　are　not　high,yet,the　serum　gastrin　level　corelatead　well　with　the　differentiation　and　clinicopathological　stage　of　colorectal　cancer.A　high　preoperative　gastrin　level　usually　denotes　high　malignancy,advanced　stage　and　poor　prognosis　of　colorectal　cancer.
