培美曲塞在胰腺癌化疗中的应用进展

摘　　要：培美曲塞是一种作用于叶酸代谢过程中多个靶点的抗肿瘤新药,主要抑制胸甘酸合酶、二氢叶酸还原酶和甘氨酰胺核苷酸转甲酰酶。基础及临床研究证实该药对包括胰腺癌在内的多种实体肿瘤有明确的抗瘤活性。该药在胰腺癌的化疗中也起到一定作用,尤其是在吉西他滨化疗失败的患者,可以作为胰腺癌的二线化疗药物。该药的不良反应主要包括骨髓抑制和皮疹等,在补充叶酸和维生素B12的情况下,不良反应明显减轻。本文就近年来该药在胰腺癌化疗中的应用进展作一简要的介绍。Pemetrexed （Alimta） is a novel muhitargeted antifolate that inhibits thymidylate synthase, dihydrofolate reductase and glycinamide ribonucleotide formyhransferase. This agent is broadly active in a wide variety of solid tumors in vivo and in vitro, including pancreatic cancer. In gemcitabine（Gem）-refractory advanced pancreatic cancer, it is a safe treatment option in second-line chemotherapy. It＇s drug-related toxicity include myelosuppression and erythra. Low level dietary supplement of folic acid and vitamin B12has significantly decreased the mucosal and bone marrow toxicity of pemetrexed without compromising its antitumor effect.
