动态脑电图监测对癫痫诊断的价值

摘　　要：为了解动态脑电图对癫痫诊断的应用价值。对112名诊断癫痫、可疑癫痫及发作性晕厥患者行24h动态脑电图(AEEG检查,并在1周内做脑电图(EEG或脑电地形图(BEAM检查作为自身对照。结果发现 AEEG 痫样放电检出率明显高于 EEG/BEAM 组(P<0.01,在癫痫发作类型中以复杂部分性发作 AEEG痫样放电阳性率明显高于 EEG/BEAM 组(P<0.05。睡眠期痫样放电检出占有痫样放电患者56/67(84%,主要出现在 NREM Ⅰ～Ⅱ期(75%。提示动态脑电图对癫痫的诊断有重要的意义。Ambulatory electroencephalography (AEEG) was employed to determine 112 patients with epilepsy,suspected epilepsy and paroxysmal syncope.The examinations of routine EEG and brain electrical activity mapping(BEAM) were also performed within one week to make a comparison in diagnosing epilepsy.The results revealed that positive rate of seizure discharge in all patients was apparently higher in AEEG group than in EEG and BEAM groups (P<0.01).In these patients with different types of epilepsy,Epileptiform discharge of complex partial seizure was more easily detected by AEEG than by EEG and BEAM (P<0.05).Seizure discharge was measured by AEEG in 56 of 67 cases during sleeping period,especially in NREM Ⅰ-Ⅱ phase.It is indicated that AEEG is valuable for diagnosing epilepsy.
