超声乳化白内障吸除术和小切口非超声乳化白内障摘除术临床效果及患者生存质量比较

摘　　要：目的:比较复明工程中超声乳化白内障吸除术(PHACO)和小切口非超声乳化白内障摘除术(SICS)的手术效果及对患者生存质量(QOL)的影响.方法:将入选的144例白内障患者随机分为两组,Ⅰ组73例,行PHACO,Ⅱ组71例,行SICS,两组均联合人工晶状体植入术.观察两组的手术效果及手术并发症;并于术前、术后3d和术后1mo使用生存质量调查量表评估两组患者的生存质量.结果:两组均获得较好的手术效果,两组术后1mo不同视力段眼数分布比较无统计学差异;两组患者术后3d和术后1mo生存质量总分和各项指标分数较术前均有显著性提高,且术后1mo优于术后3d(P<0.05).其中,自理能力、总QOL、活动能力、社交能力均有术式因素的差异(P<0.05),前两项指标Ⅰ组优于Ⅱ组,后两项指标Ⅱ组优于Ⅰ组.结论:白内障复明手术中上述两种术式手术效果及术后患者生存质量无统计学差异.SICS具有较高的性价比,值得在复明工程中及贫困地区大力推广.AIM： To compare the efficacy and quality of life （QOL） in cataract patients treated with phacoemulsification （PHACO） or manual sutureless small-incisionextracapsular cataract surgery （SICS）. METHODS： One hundred and forty-four patients with cataract were assigned randomly to be treated with either PHACO（Group Ⅰ of 73 eyes） or SICS（Group Ⅱ of 71 eyes）. Clinical outcomes and complications were observed and recorded, and QOL questionnaires were obtained respectively before surgery, and 3 d and 1 month after surgery. RESULTS： Both groups had achieved good outcomes with low complication rates. One month post operation, no significant difference was found in the visual acuity distribution between SICS group and PHACO group. The QOL and all sub-scales 3 d and 1 month post operation improved significantly in both groups, with the results of 1 month after significantly better than those of 3 d after（ P 〈 0.05 ）. The sub-scales of serf-care and total QOL in Group Ⅰ were significantly better than those in Group Ⅱ while other two sub-scales of mobility and social activities in Group Ⅱ were significantly better than those in Group Ⅰ （P 〈 0.05）. CONCLUSION： No significant difference in the efficacy and quality of life has been found in cataract patients treated with SICS or PHACO. SICS is a more cost-effective approach and may be the more appropriate and economical surgical procedure for the treatment of advanced cataracts.
