摘 要:[目的]观察中性粒细胞趋化因子(CINC)在重症急性胰腺炎(SAP)早期肺组织中表达的动态变化,探讨清胰汤(QYT)对SAP肺损伤(ALI)的保护作用及其机制。[方法]采用大鼠胰腺被膜下均匀注射5%牛磺胆酸钠0.4ml/100g制作SAP模型,随机分为假手术(SO)组,SAP组和QYT组。SAP模型制作后立即给予QYT1ml/kg灌胃,每8h重复1次。后2组又各分为6、12、24h3小组,总计7组,每组8只。观察肺组织病理学改变,肺湿重(W)/干重(D)比率,髓过氧化酶(MPO)活性,肿瘤坏死因子α(TNF-α)浓度和CINC mRNA的表达。[结果]SO组肺组织CINC mRNA有轻度表达;SAP6、12、24h各组CINC mRNA表达明显高于SO组,24h组达高峰(P<0.01),与肺W/D比率、MPO活性、TNF-α浓度及肺组织病理变化相关(P<0.05)。与SAP组比较,QYT组肺W/D比率、MPO活性和TNF-α浓度明显下降,CINC mRNA表达降低(P<0.05),肺组织病理学损害减轻,QYT24h组肺组织病理学损害减轻显著(P<0.05)。[结论]CINC是大鼠SAP早期ALI重要的炎症递质,QYT能抑制大鼠SAP肺组织中CINC的表达,从而减轻大鼠SAPALI。[Objective]To investigate the expression of the CXC chemokine cytokine-induced neutrophil chemoattractant (CINC)in severe acute pancreatitis (SAP)and to examine the effects of QingYiTang(QYT)on the course of pancreatitis-associated acute lung injury (ALI). [-Methods] SD rats induced by homogeneous injection of 0. 4ml/k9 of 5 percent sodium taurocholate beneath the pancreatic capsule were treated with QYT. After 6,12 and 24 hours,tissue edema was deter-mined by wet/dry weight ratios and myeloperoxidase(MPO)activity of lungs were measured; pathological changes in the pancreas and lungs were observed; expression of CINCmRNA and TNF-α constration in lungs were determined by reverse transeriptase polymerase chain reaction (RT-PCR) or enzyme-linked immunosorbent assay (ELISA). [Results] Compared with that in shams, wet/dry weight ratios, pulmonary expression of CINC gene, MPO activity, TNF-α concentrations in the lungs and the serum amylase were increased greatly in 24 hours(P〈0. 05). The CINCmRNA expression in lungs was significantly correlated with the severity of lung injury, MPO activity and TNF-α coneentrations in the lungs(P〈0. 05). administration of QYT significantly decreased the elevation of MPO activity,wet/dry weight ratios ,TNF-α concentrations and CINCmRNA expression in the lung(P〈0.05), and improved the histological findings(P〈0. 05). [Condusion]Overexpression of CINC mRNA in the lungs might play an important role in the pathogenesis of ALI in SAP. QYT reduced the severity of ALl, suggesting a possible application of this compound in the treatment of acute pancreatitis.
