血清肿瘤标志物CYFRA21—1，CEA，SCC，NSE，CA72—4对肺癌诊断价值的评价

摘　　要：目的探讨血清肿瘤标志物细胞角蛋白19片段（CYFRA21—1）、癌胚抗原（CEA）、鳞状细胞癌抗原（SCC）、神经元特异性烯醇化酶（NSE）、糖类抗原72—4（CA72—4）对肺癌的诊断价值。方法应用化学发光及电化学发光技术测定经病理确诊的肺癌患者（145例）、肺良性疾病患者（43例）及健康对照者（45例）血清CYFRA21—1，CEA，SCC，NSE，CA72—4水平，并计算阳性率、敏感度、特异度及准确度，采用方差分析及X^2检验的方法进行统计学分析。结果肺癌组血清五种肿瘤标志物水平显著高于肺良性疾病对照组及健康对照组（P〈0．01）。CYFRA21-1在肺鳞癌、CEA在肺腺癌、NSE在小细胞肺癌、SCC在肺鳞癌中水平最高，与其它组相比具有统计学意义（P〈0．01）；CA72—4在肺鳞癌、肺腺癌的水平显著高于小细胞肺癌（P〈0．05）。五项指标联合检测后灵敏度、准确度均显著提高（P〈0．01）。结论五种肿瘤标志物对于肺癌的辅助诊断均具有实用价值，且联合检测有助于肺癌的早期诊断和治疗。Objective To determine the diagnostic value of serum tumor marker CYFRA21-1,CEA,SCC,NSE,CA72-4 in patient with lung cancer. Methods Serum levels of CYFRA21-1,CEA,SCC, NSE,CA72-4 were measured by chemiluminescence immunoassay （CLIA） and electrochemiluminescence immunoassay （ECLIA） in 145 patients with lung cancer,43 patients with benign lung disease and 45 healthy persons. Then calculated the positive rate ,sensitivity,specificity and accuracy. Last did statistical comparison with the method of one-way ANOVA and chisquare test. Results The levels of five kinds of serum tumor marker in lung cancer group were obviously higher than healthy group（P〈0. 01）. Each tumor mark- er had its maxim value in different sub-types and was significantly higher than others,such as CYFRA21-1 to squamous cell carcinoma （P〈0.01）,CEA to adenocarcinoma（P〈0. 01）,SCC to squamous cell carcinoma （P〈0. 01）, NSE to small cell lung cancer（P〈0. 01）,CA72-4 to squamous cell carcinoma and adenocarcinoma （P〈0. 05）. The combined detection of five items could significantly improve the sensitivity and accuracy（P〈0. 01）. Conclusion These five tumor makers are valuable auxiliary parameters in diagnosing lung cancer. The combined detection is helpful to early diagnosing and treatment.
