三维适形放疗治疗晚期腹部恶性肿瘤的疗效观察及远期疗效相关因素分析

摘　　要：目的探讨三维适形放疗治疗局部晚期腹部恶性肿瘤的疗效,并分析影响远期疗效的相关因素。方法对38例晚期腹部恶性肿瘤患者进行三维适形放疗,单次剂量3～4Gy,共10～12次,3次/周或5次/周,总剂量30～48Gy,放疗结束后进行疗效评价及远期疗效相关因素分析。结果近期疗效:总有效率(CR+PR+SD)63.2%;远期疗效:3个月、6个月、12个月生存率分别为63.2%、39.5%和34.2%,中位生存期4个月;多因素分析放疗总剂量、肿块大小是影响腹部恶性肿瘤三维适形放疗远期疗效的相关因素,具有统计学意义(P<0.05)。结论(1)三维适形放疗能够缩瘤减症,提高生存质量;(2)放疗总剂量、肿块大小是影响腹部恶性肿瘤三维适形放疗远期疗效的相关因素。Objective To observe clinical treatment effect of three dimensional conformal radiation therapy on the late stage of abdominal tumor, and analyze long-term effect relevant factors influencing treatment. Methods Thirty-eight patients with late abdominal malignant tumor were treated with three dimensional conformal radiation therapy. They received 3-4Gy per day for 10-12 times,three or five times for a week with the total dosage of 30-48Gy. The treatment effect and long-term relevant factors were evaluated after radiotherapy. Results Total effective rate（CR＋PR＋SD） was 63.2%; The survival rate of 3months,6months, 12months were 63.2%, 39.5% and 34.2% respectively, MS-life span was 4months,which indicated that total dosage of radiotherapy and size of tumor were the relevant factors influencing treatment of three dimensional conformal radiation therapy on the abdominal tumor （P 〈 0.05 ）. Conclusion （ 1 ）Three dimensional conformal radiation therapy can control the tumor and improve the quality of life. （2）Total dosage of radiotherapy and size of tumor are the relevant factors influencing treatment of three dimensional conformal radiation therapy on the abdominal malignant tumor.
