摘　　要：目的采用巢式逆转录-聚合酶链反应(RT-PCR)方法检测胃癌腹腔冲洗液中癌胚抗原信使核糖核酸(CEAmRNA)的表达情况,寻求一种筛选腹腔种植转移高危患者的方法。方法采用巢式RT-PCR的方法检测69例胃癌患者和10例良性疾病患者腹腔冲洗液中CEAmRNA的表达情况,并和腹腔冲洗液细胞学(PLC)的检测结果进行比较。结果69例胃癌患者腹腔冲洗液中CEAmRNA的阳性表达率49.28%(34/69),高于PLC检测27.54%(19/69)的阳性表达率(P<0.01);10例良性病人腹腔冲洗液标本巢式RT-PCR检测CEAmRNA表达均为阴性,两组比较差异有统计学意义(P<0.01)。胃癌细胞株MNK1中巢式RT-PCR检测CEAmRNA的敏感性达到1/105。结论巢式RT-PCR是一种高敏感性、高特异性胃癌腹腔种植转移的筛选方法,优于传统的PLC方法。Objective To detect the expressions of CEAmRNA of peritoneal water in gastric cancer patients by nested RT - PCR and search a new measure to select the patients with micrometastases. Methods The results were compared through measuring the expressions of CEAmRNA by nested RT - PCR in peritoneal water in 69 cases with gastric cancer and 10 cases of benign disease. Results The positive rate of CEAmRNA was 49.28% （34/69） which higher than the positive rates of PLC in peritoneal water from gastric cancer patients （P 〈 0.01 ）. It was negative in all patients with benign disease. Comparing with gastric cancer patients, it was significant when. Conclusion Nested RT - PCR is a measure to select patients from gastric cancer patients which with higher sensitivity and specificity. It is better than traditional PLC.
