CK20 mRNA在大肠癌患者外周血微转移中的表达

摘　　要：目的检测大肠癌患者外周血中细胞角蛋白20(CK20)mRNA的表达,探讨其能否作为判断大肠癌患者外周血微转移及预后的分子生物学指标。方法采用RT-PCR技术检测32例大肠癌患者、11例肠道良性病变患者和20例健康体检者外周血中CK20 mRNA定性和半定量表达。结果(1)CK20 mRNA在大肠癌患者中的阳性率为50.00%,相对系数为(0.89±0.17);在肠道良性病变患者中阳性率为9.09%,相对系数为0.11;在健康体检者中阳性率及相对系数均为0。大肠癌患者与肠道良性病变患者、健康体检者相比,CK20 mRNA明显增高(P<0.05)。(2)CK20 mRNA阳性率和相对系数在不同临床分期、远处转移中差异有统计学意义(P<0.05);其阳性率在浸润深度、淋巴结转移中无统计学意义,但相对系数有统计学意义(P<0.05)。阳性者生存率低于阴性者(P<0.05)。(3)CK20 mRNA与癌胚抗原(CEA)、糖链抗原199(CA199)表达呈正相关(P<0.05)。结论CK20mRNA的表达能预测大肠癌患者外周血微转移,可判断预后。Objective To detect the expression of cytokeratin 20 （CK20） mRNA in peripheral blood of patients with colorectal carcinoma and to discuss whether it can be used as molecular biological markers of predicting micrometastasis in peripheral blood and evaluating prognosis. Methods Peripheral blood of 32 colorectal carcinoma patients, 11 benign colorectal lesion patients and 20 healthy examination cases were detected by RT - PCR technique for qualitative and semi - quantitative analysis of CK20 mRNA. Results The positive rate of CK20 mRNA in patients with colorectal carcinoma was 50.00%, relative coefficient was 0. 89 ＋ 0. 17 ; and the positive rate of CK20 mRNA was 9.09% in benign colorectal lesion, relative coefficient was 0.11. CK20 mRNA was not detected in healthy people. Comparing the benign colorectal lesion patients and healthy people, the expressions of CK20 mRNA in colorectal carcinoma patients were significantly increased （P 〈 0.05 ）. The positive rates and relative coefficients of CK20 mRNA were significantly correlated with clinical stage, distant metastasis （ P 〈 0.05 ） ; but the positive rate of CK20 mRNA was not correlated with depth of tumor invasion, lymph node metastasis, relative coefficient was correlated with them （P 〈 0.05 ）. The patients with positive expression have lower survival rates compared with negative expression of patients （P 〈 0.05 ）. The positive rates and relative coefficients of CK20 mRNA were positively correlated with the expression of serum carcinoembryonic antigen （CEA） and carbohydrate antigen 199 （CA199） level in colorectal carcinoma patients. Conelution CK20 mRNA could predict micrometastasis in peripheral blood and evaluate prognosis in colorectal carcinoma patients.
