摘  要:目的探讨肺癌CT引导下穿刺活检术中CT辐射剂量与活检准确率的相关性。方法MX8000多层螺旋CT引导下栅条法定位,60例肺部肿块临床拟诊肺癌患者平均分为两组,30例常规剂量(120kv,120mAs),30例低剂量(120kv,30mAs),比较两组患者图像质量、CTDI及采用卡方检验比较两组穿刺的准确率。结果两组患者两种不同剂量120mAs,30mAs下显示病灶的形态、边缘无明显差异,病灶密度不均及短毛刺显示,低剂量组不及常规剂量组。常规剂量组每位患者总的CTDI约24.3mGy,低剂量组每位患者总的CTDI约8.4mGy,低剂量组CTDI约为常规剂量的1/3。30例常规剂量和低剂量组最后分别有27例和26例确诊为肺癌,穿刺诊断准确率分别为90.0%和86.7%,差异无统计学意义,即两种剂量条件穿刺,两组病理诊断准确率无统计学意义。结论低剂量CT的图像质量可以满足CT引导的目的,肺癌低剂量CT引导穿刺活检术,不仅保证了穿刺的准确率,还降低了患者的辐射剂量和CT球管的损耗。Objective To explore the dependablity of radiation dose and the diagnostic accuracy of CT-guided lung biopsy. Methods The punctured location was comfirmed by grid on MX8000 multislices CT scan. 60 patients who were suspected lung cancer were divided into two groups. In group 1, routine radiation dose(120kv, 120mAs) was used in 30 patients. But in group 2, low radiation dose (120kv, 30mAs) was used in another 30 patients. To compare the image quality, CTDI and accurate rate between two groups. Results There were no differences on the morphous and borderline of the lung tumors between two groups. The images of routine dose demonstrate the homogenous density and shotrt spike more clearly than those images of low dose. The CTDI of every patient in group 1 was about 24.3mGy, but 8.4mGy in group 2. So the radiation dose of group 2 to that of group 1 was 1/3.27 cases in group 1 and 26 cases in group 2 were comfirmed lung cancer by histopathology. The diagnostic accuracy was 90.0% and 86.7% respectively. The difference was no statistical significance. Conclusion The image quality of low dose CT scan can meet the purpose of CT-guided lung biopsy. It can not only keep the diagnostic accuracy of the puncturation of lung cancer, but aslo cut down the radiation dose of the patient and loss of the tube.
