70岁以上高龄胃肠道肿瘤围手术期处理(附72例临床分析)

摘　　要：目的探讨总结高龄胃肠道肿瘤患者围手术期处理方法。方法回顾性分析经手术治疗的72例70岁以上胃肠道肿瘤患者的病例资料。结果72例中61例合并有1—5种不同类型的并存病。急诊手术12例，限期手术50例；根治手术51例，姑息手术11例。术后33例出现了并发症，其中28例出现2种或2种以上的并发症，最多1例出现6种并发症。围手术期死亡1例。结论高龄胃肠道肿瘤患者常并存一种或多种不同类型的并存病，增加了外科手术的危险性。加强围手术期管理对有效降低病死率，提高外科治疗的成功率有重要意义。Objective To explore the perioperative management of gastrointestinal tumor in geriatric patients. Methods Data of 72 aged patients （ 〉 70 years old） received operations for gasrointestinal tumors were analyzed retrospectively. Results Sixty-two patients with 1 -5 coexisting diseases had 12 emergency operations and 50 date-choose ones, or 51 radical operations and 11 palliative ones. Postoperative complications were found in 33 cases and among them, 28 patients had two or more kinds of complications and one patient had six ones. One patient died in perioperative period. Conclusion Coexisting diseases in geriatric patient with gastrointestinal tumor added the risks for the surgical procedures. It is significant that proper perioperative management will improve surgical treatments and reduce complications.
