摘  要:回顾性分析191例数字乳腺影像报告和数据系统(BI-RADS)4级病灶,共有77例病变最终获病理证实。对照影像学特征与病理结果,通过卡方检验得出,良、恶性病变的肿块边界和钙化形态差异存在显著性意义,P值分别为0.032和0.004。年龄,肿块形态、密度,钙化分布,结构扭曲等表现在两组间未见显著性差异(P>0.05)。运用Logistic回归模型评估影像学特点与病理结果的相关性发现,仅钙化形态与良恶性结果有显著相关性,余影像学表现与恶性结果均无显著相关性。通过该影像-病理对照分析,为建立明确的BI-RADS4的3亚组分类标准提供重要的参考依据。To evaluate radiological findings of Breast Imaging Reporting and Data System (BI-RADS) category 4 and to correlate them with histological features, images of 191 patients were analyzed retrospectively and histologically, and 77 of them were identified as pathological mammographic images. Mass border and calcification types see significant differences between benign and malignant cases according to chi square test, with P=0.032 and 0.004, respectively. Age, mass shape, density, calcification distribution, and structure distortion see no significant difference. The study also shows that there is a significant correlation between fine pleomorphic calcifications and malignant outcome by using a logistic regression model. And the residual radiological features see no correlation with the outcome. This study provides a basis in subdividing Category 4 into 4a, 4b and 4c.
