扶贫白内障复明手术2321例效果分析

摘　　要：目的总结在贫困地区的乡镇医院开展白内障复明手术的效果及体会。方法对汕头大学医学院扶贫眼科手术队在医疗设备、条件差的乡镇医院对贫困白内障患者实施免费白内障摘除并人工晶体植入手术复明2321例的效果与并发症进行分析。结果手术后追踪复查2103例,术后1w视力0.5以上1219例(57.96%);术后一个月视力0.5以上1628例(77.41%);视力0.8以上1240例(58.96%);视力0.25以下105例(4.99%)。术后并发症:角膜水肿189例(8.14%);后囊膜破裂158例(6.81%);迟发性葡萄膜炎65例(2.80%);前房出血、玻璃体出血7例;人工晶体纤维膜包裹2例。结论汕头大学医学院扶贫眼科手术队在贫困地区的乡镇医院开展白内障复明手术临床疗效较好。条件差的乡镇医院施行白内障复明手术必须由经验丰富的医生主刀手术,尽可能改善手术设备,减少、减轻并发症。同时,要培训当地医务人员,以保证手术后正确的治疗与护理。Objective To summarize the effect and impression of the cataract surgeries in small town＇ s hospitals in poverty - stricken zone. Methods To analyze the effect and complication of the cataract surgeries which remove the pathological changes and implant Artificial lens for vision rehabilitation on 2321 cases. The cataract surgeries were enforced in the poorly equipped small town＇ s hospitals by the poverty alleviation ophthalmologic operation team of Shantou University Medical College and free to the poverty patients. Results Following - up and rechecking 2103 cases after the surgeries, 1219 cases improved their visions above 0. 5 in one week after the surgeries （57.96%） ; 1628 cases improved their visions above 0. 5 in one month after the surgeries （77.41%） ; 1240 cases improved their visions above 0. 8 after the surgeries （8.96%） ; 105 cases＇ visions were below 0. 25 after the surgeries （4. 99% ）. The complications ：189 cases were complicated with corneal edema （8.14%） ; 158 eases were complicated with posterior- capsule rupture （6. 81% ） ; 65 cases were complicated with tardive uveitis （2. 80% ）＇; 7 cases were complicated with hyphema and vitreous hemorrhage; 2 cases＇ Artificial lens were surrounded with textile 6ber membrane. Conclusion The effect of cataract surgeries enforced in the small towns＇ hospitals in poverty - stricken zone by the poverty alleviation ophthalmologic operation team of Shantou University Medical College is good. The cataract surgeries enforced in the poorly equipped small towns＇ hospitals should be operated by seasoned doctors and improve the surgical apparatus, reduce the complication as far as possible. At the same time, training the local medical workers to ensure that the patients receive correct treatment and nursing after the operation.
