Ⅰ型自身免疫性肝炎增殖诱导配体、B淋巴细胞刺激mRNA的表达与临床特点

摘　　要：目的检测Ⅰ型自身免疫性肝炎(autoi mmune hepatitis typeⅠ,AI H-Ⅰ)患者增殖诱导配体(aproliferation-inducing ligand,APRIL)和B淋巴细胞刺激因子(B-lymphocyte activating factor,BAFF)mRNA水平,研究APRIL和BAFFmRNA的表达与AI H-Ⅰ临床特征之间的关系。方法半定量PCR法检测37例Ⅰ型AI H确诊患者(Ⅰ型AI H组)、19例乙型肝炎患者(乙型肝炎组)、15例正常人(正常对照组)外周血单核细胞APRIL和BAFFmRNA的表达水平,同时检测Ⅰ型AI H患者外周血中IgG、C3、C4、抗核抗体(ANA)、抗平滑肌抗体滴度(ASMA)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、球蛋白(GLO)、C反应蛋白(CRP)水平。结果APRIL、BAFFmRNA在Ⅰ型AI H组的表达水平(0.65±0.39,0.76±0.35)显著高于乙型肝炎组(0.33±0.26,0.37±0.24)和正常对照组(0.30±0.25,0.23±0.20)。APRIL和BAFFmRNA表达水平在Ⅰ型AI H、乙型肝炎组、正常对照组中均呈中等强度的正相关(r=0.418,P<0.05)。APRIL、BAFFmRNA表达水平与疾病活动度指标(ALT、AST、GLO、IgG、CRP)、肝组织学炎症与纤维化评分之间无明显相关(P>0.05)。结论Ⅰ型AI H患者的APRIL、BAFFmRNA的表达水平显著升高,其水平与患者的预后有一定的相关性。Objective To evaluate the serum levels of a proliferation-inducing ligand（APRIL） and B-lymphocyte activating factor（BAFF） in patients with autoimmune hepatitis type-I （AIH I ） and determine their relations to the clinical features of AIH- I . Methods Serum APRIL and BAFF levels in 37 patients with AIH- I （AIH- I group）, 19 patients with chronic hepatitis B（HBV group）, and 15 healthy subjects （normal control group） were examined using semi-quantitative polymerase chain reaction. Liver function tests, quantitative immunoglobulin, C reactive protein （CRP）, C3, C4, antinuclear antibody（ANA）, anti smooth muscle antibody （ASMA） titres and liver biopsy were also assayed in AIH- I patients. Results APRIL and BAFF levels in the serum samples from AHI- I group （APRIL： 0. 65±0. 39,BAFF：0. 76±0. 36） were significantly higher than those from HBV group （APRIL..0.33±0.26,BAFF.. 0. 37±0. 24） and those from normal control group （APRIL..0.30±0.25,BAFF：0.23±0.20）（ P 〈0.05）. There was a median positive correlation between BAFF and APRIL in AIH- I group, HBV group and normal control group （ r=0.418, P 〈0.05）. However, disease activity parameters （ALT, AST, GLO, IgG, CRP） and histological staging （inflammation and fibrosis score） did not correlate with serum APRIL and BAFF levels （ P 〉 0.05）. Conclusion Serum APRIL and BAFF levels are higher in the patients with AIH- I. Serum APRIL and BAFF levels do correlate with prognosis.
