摘 要:目的研究WT1基因在急性白血病(AL)患者骨髓细胞中的表达及其临床意义。方法采用实时定量RT-PCR方法动态检测92例初治AL患者WT1基因的表达。结果初治AML各亚型中M5基因表达阳性率最高,为91.7%,其WT1基因中位表达水平最高,达1308.6,显著高于其他亚型,差异有统计学意义(P<0.05);初治ALL各亚型(L1,L2,L3)之间WT1基因表达阳性率及中位表达水平差异无统计学意义(P>0.05);AL患者中,WT1基因表达阳性组CR率显著低于阴性组,阳性组缓解后半年内复发率显著高于阴性组,差异均有统计学意义(均P<0.05);初治组、复发组及未缓解组AL患者骨髓细胞WT1基因中位表达水平明显高于完全缓解组和对照组,差异有统计学意义(P<0.05)。结论WT1基因在AL患者骨髓细胞中表达水平可作为临床评估疾病的发生发展、判断预后及检测微小残留病(MRD)的指标。Objective To study expression of Wilms' tumor gene (WT1) in acute leukemia(AL) cells and its clinical significance. Methods Real-time quantitative reverse transcription polymerase chain reaction(RQ-RT-PCR) method was established for detecting dynamically WT1 expression levels in bone marrow cells in 92 cases of de novo AL. Results Positive rate of WT1 expression of de novo AML M5 sub-type was the highest (91.7%);Median expression level of WT1 in M5 sub-type of AML was the highest (1308.6). It was significantly higher than that in the other sub-types of AML (P〈0.05);There was no significant difference in positive rate and median expression level of WT1 among sub-types(L1, L2, L3) of de novo ALL;CR rate of positive group of WT1 expression of AL patients was significantly lower than that in the negative group (P〈0.05); Relapse rate of 6 months after remission in the positive group was significantly higher than that in the negative group (P〈0.05); Median expressions of WT1 in bone marrow cells of AL patients in de novo group,recurrence group and non-relieved group was significantly higher than those in the CR group and the control group (P〈0.05). Conclusion Expression of WT1 in bone marrow cells of AL patients can be a marker to evaluate occurrence,development and prognosis of the disease and to test minimal residual disease (MRD).
