摘  要:目的探讨nm23基因产物和细胞外基质抗原的表达与声门上型喉鳞癌颈淋巴结转移的关系。方法分别应用nm23/NDPK多克隆抗体、抗Ⅳ型胶原纤维(Type IV Collagen,CTⅣ)单克隆抗体、抗层粘连蛋白(Laminin,LN)单克隆抗体、抗纤维连接蛋白(Fibronectin,FN)单克隆抗体以链霉素亲生物素过氧化酶免疫组织化学法观察nm23/NDPK和这三种抗原在72例声门上型喉鳞癌中的表达分布情况及其与颈淋巴结转移的关系。统计学处理采用x2检验。结果声门上型喉鳞癌中nm23/NDPK阴性者发生颈淋巴结转移率高于表达阳性者,二者之间有较强相关性(P<0.01)。CTⅣ、LN、FN表达弱阳性者发生颈淋巴结转移率高于强阳性表达者,也表明其间有较强相关性(P<0.01)。结论nm23/NDPK、CTⅣ、LN、FN表达情况可作为预测声门上型喉鳞癌发生颈淋巴结转移的重要参数之一。Objective To investigate the relationship of the expressions of nm23/NDPK, 1V collagen (CtiV), laminin (LN) and fibronectin (FN) with the metastasis of cervical lymph node in laryngeal spuamous cell carcinomas. Methods An immunohistochemical study was performed using the polyclonal antibody against nm23/NDPK and monoclonal antibodies against human type IVcollagen, laminin and fibronectin to observe the expression and the distribution of nm23/NDPK , CTIV, LN and FN in 72 cases of supraglottie laryngeal squamous cell carcinomas and to assess their relationship with the metastasis of cervical lymph node. x2 test was applied for statistic analysis. Results There was a higher risk of cervical lymph node metastasis in cases with negative expression of nm23/NDPK than in those with positive nm23/NDPK expression ( P 〈 0.01). There was also a higher risk of cervical lymph node metastasis in cases with weak positive expression of CTIV, LN and FN than in those with strong positive expression of CTIV, LN and FN ( P 〈 0.01). Conclusion The expressions of nm23/NDPK ,CTIV ,LN and FN seems to be helpful to prognosticate the frequency of cervical lymph node metastasis in laryngeal squamous cell carcinomas.
