摘  要:目的:探讨MRI对甲状腺相关眼病(thyroid associated ophthalmopathy,TAO)球后放射治疗疗效预测的价值。方法:回顾性分析64例行球后放射治疗的TAO患者的临床资料,所有患者治疗前已行MRI检查,并且为新发、未行皮质激素治疗、TAO病程≤1年、临床活动评分(CAS)≥4分者。随访12个月进行疗效评价,分为有效组、无效组。分析两组TAO患者MRI影像资料与疗效的关系。结果:有效组50例,MRI提示眼外肌均受累,无效组14例,其中MRI提示眼外肌受累10例,两组间差异有统计学意义(P=0.002);有效组的受累眼外肌T2信号强度比值明显高于无效组(P<0.001);受累眼外肌组内不同的T2信号强度分级间的疗效差异有统计学意义,信号强度比值愈高,疗效愈好。结论:MRI可以作为TAO球后照射治疗疗效预测的重要手段。Objective To investigate the value of magnetic resonance imaging (MRI) in predicting the therapeutic outcome of orbital radiotherapy for thyroid associated ophthalmopathy (TAO). Methods Data of 64 patients with new diagnosed TAO, whose clinical activity score (CAS) was not less than 4 and duration of TAO was not more than 1 year, and who had received MRI detection but had not received hormone treatment, were retrospectively analyzed. After 12 months of follow-up, all the patients were divided into effective group or ineffective group, and the relation between MRI imaging and therapeutic effects was analyzed. Results Effective group included 50 patients, ineffective group included 14 patients. Enlarged extraocular muscle shown by MRI were 50 vs 10 between effective group and ineffective group (P = 0.002). MRI-T2 signal intensity ratio (SIR) of extraocular muscle was much higher in effective group than that in ineffective group (P 〈 0.001 ). And there were significant differences among MRI-T2 SIR classification groups, the higher the signal SIR, the better the therapeutic effect(P 〈 0.001 ). Conclusion MRI is an effective mode to predict therapeutic outcome of orbital radiotherapy for TAO.
