间歇性外斜视手术治疗的体会

摘　　要：用同视机查双眼单视功能，Titmus查近立体视锐，手术量按外直肌后徙1mm矫正2△-3△，内直肌截除1mm矫正4△-5△计算，根据AC／A值及双眼视功能，3例行单眼外直肌后徙书术，8例双眼外直肌后徙术，3例行内直肌缩短术，35例行单眼外直肌后徙加内直肌缩短术，1例行双内直肌缩短加外直肌后徙术，16例行双外直肌后退加内直肌缩短术，5例行下斜肌切断并部分切除术，6例行内外直肌移位术，41例局麻下手术，25例全麻下手术。66例中完全功能治愈33例，临床治愈25例，总治愈者58例，治愈率87．8％。间歇性外斜视应正确选择手术时机，采取合理的手术方案，有利于双眼单视功能的建立。Objective To investigate the surgical juncture and effect in intermittent exotropia. Meth- ods The surgical decision was made by the guidelines of 1 mm lateral rectus resection equal to 2-3 prism diop- ters and 1 mm of medical rectus resection equal to 4-5 prism diopters. Examine binocular single vision with syn- optophore and stereoacuity with Titmus. According to the rat of AC/A and binocular vision. 66 cases received surgery which included single lateral rectus recession in 3 cases, both lateral rectus recession in 8 cases, single medial rectus resection in 3 cases, single lateral rectus recession combined medial rectus resection in 35 cases, both medial rectus resection combined with single lateral rectus recession in 1 case, both lateral rectus recession combined single medial rectus resection in 16cases. Local anesthesin was used in 41 cases, and general anesthe- sia used in 38cases. Results Cases（58 ） got ocular alignment after suraery. Conclusion For the treatment of intermittent exotropia juncture and reasonable scheme of sugery is benifieal to the binocular single vision recovery.
