摘　　要：目的：探索一种新的微创心脏瓣膜替换术。方法：作者于１９９７年３月至１９９７年７月为２６例风湿性心脏病患者和１例感染性心内膜炎患者经右侧腋下胸部小切口，完成了心脏瓣膜替换手术。结果：患者手术皮切口长度一般在７．５～１０．０ｃｍ之间，平均８．６±１．６ｃｍ。其中经第４肋间完成２１例，第３肋间完成６例。所有患者均康复出院。结论：该项技术不仅可减轻手术对患者的损伤，同时为换瓣手术技术的改进提供了新经验，拓宽了应用范围。? Objective:To develop a new minimally invasive approach for heart valve replacement.Methods:Through right minimal vertical infraaxillary thoracotomy incision(by intercostel space),26 patients with rheumatic heart disease and 1 patient with subacute endocarditis underwent heart valve replacement from March to July,1997. Results:twenty one Cases were through the forth intercostal space,6 cases were through the third intercastal space.The length of the incision were about 7.5—10.0 cm in length with a mean of 8.6±1.6 cm.No patients died during or after operation,all patients got a good result and discharged from the hospital earlier than routinely. Conclusion:This minimally incisive valve operation is a new,safe,small wounded and cosmetic approach for heart valve replacement.
