脑卒中偏瘫早期康复疗效观察

摘　　要：目的探讨早期基本康复训练对脑卒中患者运动功能、平衡功能、日常生活活动（ＡＤＬ）和继发性残疾的影响。方法对２００例脑卒中偏瘫患者以抽签方式随机分为康复组（１００例），对照组（１００例）。对康复组进行了早期康复研究，平均于病后９天开始，应用简单的康复医疗程序，进行床边治疗。结果平均３５天治疗后，取得明显疗效：①运动功能达ＢｒｕｎｎｓｔｒｏｍⅣ级以上者，上肢４９％、手４４％，下肢７５％。步行恢复率达６３％；②坐位平衡功能达Ⅱ级和Ⅲ级者占８３％，站位平衡占５０％；③日常生活活动积分在６０分以上、恢复良好者达５４．３％；④预防及减少了继发性残疾。与对照组相比存有显著差异（Ｐ＜０．０００５）。Objective This study lays emphasis on basic training effect of early rehabilitation on limb movement,balance function, activities of daily living (ADL) and secondary disability of stroke hemiplegia.Methods 200 patients were randomly divided into two groups, rehabilitation group(100 patients ) and control group(100 patients ) by drawing lots. The patients of rehabilitation group were trained with early basic rehabilitation training procedure on average the ninth day after attack.Results After average 35 days'training, rehabilitation group showed significant improvement in comparison with the control group:① Motor functions(Brunnstrom 6 grades) that improved above IV of Brunnstrom accounted for 49 percent in upper extremity,44 percent in hand and 75 percent in lower extremity.Walking recovery rate took 63 percent;②Balance function that improved toⅡand Ⅲ grade accounted for 83 percent in sitting balance , 50 percent in standing balance; ③ADL that improved to 60 points accounted for 54.3 percent;④Secondary disability was prevented and decreased. All the indexes showed significant differences with those of the control group(P< 0.0005 ).Conclusion Early stroke rehabilitation by simple rehabilitation procedure is effective and practical to stroke.
