摘 要:目的:前期研究发现POSTN mRNA表达在乳腺癌的淋巴结转移癌中较其配对的原发癌下调,是潜在的乳腺癌转移相关基因。本研究进一步通过大样本临床病例验证,阐明乳腺癌组织中POSTN mRNA水平与乳腺癌临床病理因素的关系,并评估其预测患者预后的临床价值。方法:采用实时定量RT-PCR方法检测174倒乳腺浸润性导管癌原发癌组织中POSTN mRNA水平;采用ROC(Receiver Operating Characteristic)曲线确定POSTN mRNA水平分组的阈值;X2检验比较组间差异;Kaplan-Meier法绘制生存曲线;Log-rank时序检验比较组间生存期的差异。结果:POSTNmRNA水平在雌激素受体(ER)阴性乳腺癌组织中低于ER阳性组织(P=0.007),在其它临床病理因素各组间无统计学差异。ER阳性乳腺癌患者POSTN mRNA低水平组3年无瘤生存率和无转移生存率均低于高水平组(P=0.052,P=0.033),而在其它临床病理因素各组中POSTN mRNA状态与患者预后无关。结论:POSTN mRNA高表达是ER阳性乳腺癌的生物学特征之一,且POSTN mRNA低表达是ER阳性乳腺癌患者早期复发转移的危险指征。Objective: Previous studies have found that the POSTN mRNA level is down-regulated in metastatic lymph node tissues of breast cancer compared to the paired primary cancer. We considered that POSTN might potentially be related to the metastasis of breast cancer. The purpose of this study was to clarify the correlation between the mRNA level of POSTN and clinicopathologic factors in breast cancer, and to evaluate its clinical value in predicting the prognosis of breast cancer patients. Methods: Real-time reverse transcription-polymerase chain reaction (RT-PCR) was used to detect the mRNA expression levels of POSTN in 174 cases of primary invasive ductal carcinoma. A Receiver Operating Characteristic (ROC) curve was used to identify the cut-off value for POSTN mRNA level to group the patients. A chi square test was used to compare the differences between groups. Survival analysis was carried out using the Kaplan-Meier method. Log-rank test was used to compare the differences in survival between groups. Results: POSTN mRNA levels in estrogen receptor (ER) negative breast cancer tissues were lower than those in ER positive breast cancer tissues (P=-0.006). No significant difference in POSTN mRNA levels was found between groups. In patients with ER positive tumors, the cases with low POSTN mRNA levels had a shorter 3-year disease-free survival and metastasis-free survival than the cases with high POSTN levels (P=0.052, 0.033). No correlation was found between POSTN mRNA level and prognosis. Conclusion: High POSTN mRNA level is one biological characteristic of ER positive breast cancer. Low expression of POSTN mRNA predicts high risk of recurrence and distant metastasis at earlier period after diagnosis for ER positive breast cancer.
