联合应用谷氨酰胺和生长激素强化营养对喉癌患者术后蛋白代谢和免疫功能的影响

摘　　要：目的:研究谷氨酰胺和重组人生长激素强化营养联合应用对喉癌患者术后蛋白代谢、免疫功能的影响。方法:将64例喉癌患者随机分为对照组和谷氨酰胺+生长激素组,两组均于术后给予10d胃肠外营养,谷氨酰胺+生长激素组按每天0.4g/kg的剂量加用谷氨酰胺,同时还皮下注射8IU/d重组人生长激素(珍怡),于手术前及手术后d10测患者体重,手术伤口愈合情况及血清白蛋白水平,T细胞亚群、IgA、IgG、IgM,并与对照组进行比较。结果:谷氨酰胺+生长激素组的体重与手术伤口愈合均较对照组好(P<0.05),谷氨酰胺+生长激素组的血清白蛋白、IgA、IgG在术后d10均较对照组有显著的改善(P<0.01)。但IgM及T细胞亚群改变不明显(P>0.05)。结论:喉癌患者在术后常规营养支持治疗的基础上,联合应用谷氨酰胺加重组人生长激素可以更好地改善患者术后的蛋白代谢及提高免疫水平,促进术后病情恢复。Objective： To investigate the effect of combined supplementation of glutamine and nant human growth hormone on protein metabolism and immunologic function in postoperative patients with laryngocarcinoma. Method： Sixty -four patients with laryngo -carcinoma were randomly divided into control （ n = 32 ） and Glu plus rhGH （ Glu ＋ rhGH, n = 32 ） group. All patients were given parenteral nutrition after operation for ten days. The patients in Glu ＋ rhGH group were given alanylglutamine solution at a dose of 0.4g/kg and injected of rhGH 8IU every day. The level of weight, surgical wound healing, serum al- bumin （ALB ） , T lymphocyte subgroups, IgA, IgG, IgM were observed. Result： The levels of weight, sur- gical wound healing, ALB, IgA and IgG in Glu ＋ rhGH group on the tenth day after operation were higher than those in the control group （ P 〈 0.05 ）. But there was no significant difference in IgE and T lymphocyte subgroups between two groups （ P 〉 0.05 ）. Conclusion ： Combined supplementation of glutamine and recombinant human growth hormone as enhanced conventional nutritional support therapy can improve the postoperative protein metabolism and immunologic state, promote the restoration of condition in patients with laryngo - carcinoma.
