摘 要:目的探讨细胞系列特异性抗原在急性白血病(AL)中表达的差异及其临床意义。方法采用CD45/SSC双参数设门流式细胞术检测183例AL患者幼稚细胞表面及胞浆抗原表达。结果在AML中,细胞膜抗原CD13、CD33、CD117表达最常见,分别为95.0%、95.7%和81.4%,但CD117特异性高于CD13和CD33(P<0.01);胞浆抗原cMPO敏感性和特异性与CD117比较,差异无统计学意义(P>0.05);在B-ALL中胞膜抗原CD19敏感性高于CD20(P<0.01),两者特异性比较,差异无统计学意义;胞浆抗原cCD79a敏感性与CD19比较,差异无统计学意义(P>0.05),但特异性高于CD19(P<0.01);T ALL中胞膜抗原CD7敏感性较高,达95.0%,但特异性仅为70.8%,胞浆抗原cCD3敏感性和特异性分别为85.0%和95.7%。结论多参数流式细胞术检测细胞系列特异性胞膜/胞浆抗原表达是诊断AL的一项可靠技术。Objective To investigate the differential expression of serial specific antigens in pa- tients with acute leukemia (AL) and its clinical significance. Methods Multi-parameter flow cytometry and CD45/SSC gating were used to analyze the surface and cytoplasmic antigen expression in juvenile cells of 183 AL patients. Results CD13, CD33 and CD1l7 were the most commonly expressed surface antigens in acute myeloid leukemia (AML) patients (95.0%, 95.7% and 81.4% respectively). The specificity of CD117 was higher than that of CD13 and CD33 (P〈0.01). There were no significant differents in sensitivity and specificity between cytoplasmic antigen MPO and surface antigen CD117 (P〉0. 05). In B-lineage acute lymphocytic leukemia (ALL), the sensitivity of CD19 was higher than CD20, but the specificity was similar to CD20. cCD79a showed high sensitivity and specificity. In Tlineage ALL, the sensitivity of CD7 was high (95.0%), but the specificity was low (70.8%). But the sensitivity and specificity of cCD3 was 85.0% and 95.7% respectively. Conclusion Muli-parameter flow cytometry is a reliable technique in monitoring the expression of lineage-related surface/cytoplasmic antigens in the diagnosis of acute leukemia.
