摘  要:目的检测胰腺癌患者胰液中人Hedgehog相互作用蛋白(human hedgehog interacting protein,HHIP)基因CpG岛甲基化状态,探讨其意义。方法收集2006年至2008年82例行内镜下胰胆管逆行造影术(ERCP)患者的胰液。通过巢式甲基化特异性聚合酶链反应(nested methylation specific PCR,NMSP)检测31例胰腺癌、36例慢性胰腺炎(CP)及15例良性非胰腺疾病患者(对照组)胰液中HHIP基因CpG岛甲基化状况,并随机取各组中的1例进行测序。结果胰腺癌、CP及对照组患者胰液中HHIP基因CpG岛甲基化阳性率分别为54.8%(17/31)、2.8%(1/36)和6.7%(1/15),胰腺癌组的甲基化阳性率显著高于其他两组(P〈0.01)。测序结果显示胰腺癌的HHIP基因有8个CpG位点发生甲基化,而CP及对照组的HHIP基因CpG位点未发生甲基化。结论胰腺癌患者胰液中的HHIP基因CpG岛发生高甲基化,检测胰液中HHIP基因甲基化状态可能作为胰腺癌诊断的靶点。Objective To investigate the CpG islands methylation status of human hedgehog interacting protein (HHIP) gene in pancreatic juice and to investigate the clinical significances. Methods Pure pancreatic juice (PP3) was collected from 82 patients by endoscopic retrograde cholangiopancreatography (ERCP) from 2006 to 2008. The CpG islands methylation status of PPJ from 31 patients with pancreatic carcinoma (Pca), 36 patients with chronic pancreatitis (CP) and 15 patients with benign non-pancreatic disease (NP) were examined by nested methylation specific PCR (NMSP), and then 1 random case was selected from each group for cloning and sequencing. Results The methylation rates of HHIP in PPJ of PCa, CP and NP were 54.8% ( 17/31 ) , 2.7% (1/36) and 6.7% ( 1/15), respectively, and the methylation rate in PCa group was significantly higher than those in CP and NP groups ( P 〈 0.01 ). Eight CpG sites methylation was observed in PCa by cloning and sequencing, while no CpG sites methylation was observed in CP and NP. Conclusions CpG island hypermethylation of HHIP was detected in pancreatic juice, which may be a useful marker in the diagnosis of PCa.
