荧光原位杂交技术在乳腺癌HER2的表达及其意义

摘  要:目的探讨荧光原位杂交(FISH)与免疫组织化学(IHC)检测乳腺癌人表皮生长因子受体2(HER2)基因状态中的相关性和差异性。方法对40例原发性乳腺癌组织做石腊切片,分别用FISH和IHC检测HER2基因扩增和HER2蛋白表达,以FISH检测结果为标准,评价IHC检测的基因扩增比率。结果IHC检测HER2蛋白表达(3+)的基因扩增比率为88.9%(8/9),1例无扩增者为17号染色体多体;IHC(2+)的基因扩增比例为71.4%(5/7),2例无扩增者中有1例为17号染色体多体;IHC(1+和0)的无基因扩增。结论IHC可以做为HER2基因状态检测的初筛;IHC(2+)与IHC(3+)存在假阳性,与17号染色体多体有关,故应该做FISH检测确诊。Objective To probe into the restdts of fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC) in detection of genetic state of epidermis growth factor receptor 2 (HER2) in breast cancer pateints. Methods Paraffin wax shoe of 40 cases of primary breast cancer tissues was prepared and HER2 gene was amplified and HER2 albumen expression was detected with FISH and IHC separately. Results The expression rate o HER2 albumen by IHC (3+) was 88.9% (8/9) . The expression rate of HER2 albumen by IHC(2+) was 71.4% (5/7),2 cases were not amplifiedt one of them had No.17 chromosome multimer. The IHC ( 1+ and 0) had not amplification. Conclusion IHC can be used for initial screening of HER2,while IHC (2+)and IHC (3+)might give false positive associated with No.17 chromosome multimer. Thus FISH detection is necessaryu for a definite diagnosis.
