中心静脉导管留置引流并心包内灌注顺铂、鸦胆子油乳治疗恶性心包积液58例临床观察

摘　　要：目的观察心包内中心静脉导管留置引流并灌注药物治疗恶性心包积液的疗效。方法对58例恶性心包积液患者应用中心静脉导管心包穿刺置管引流,将顺铂(DDP)及鸦胆子油乳(Brucea javanicaoil emulsion,BJOE)稀释后注入心包腔内治疗,每周2次。结果完全缓解(CR)26例,部分缓解(PR)25例,总有效率(CR+PR)为87.9%,毒副反应轻,主要为轻度消化道反应和一过性发热胸痛。结论该方法简便易行、安全,腔内给药副作用低,能有效控制恶性心包积液,值得临床推广使用。Objective To observe the effect of treating malignant hydropericardium by drain and perfusion medicine therapy through central venous catheter. Methods Paracentesis were put on 58 cases of malignant hydropericardium patients by central yenous catheter, inflused attenuated DDP and B JOE to cavitas pericardialis,twice every week. Results 26 cases were complete remission, 25 cases were partial remission,total effective rate is 87. 9%, adverse reaction mainly with slightly enteron reaction and transient fever and chest complaint. Conclusion Intracavitary administration is convenient and safety.It also can control malignant hydropericardium effective with slight side effect and deseverd to be generalization on clinical application.
